Document the History Review

You can document the review details of a patient's history in the generated encounter document. You can also document the review status of medical, surgical, family, and social histories on the template. Open NextGen® Enterprise EHR. On the Histories template, select the History Review link.The History Review template opens. In Med/Surg/Interim Hx, Family, and Social, select the applicable option. Detailed document allows you to view all the details entered in the Histories template in the encounter document. Reviewed, updated allows you to indicate that you have reviewed and updated the patient's histories. Reviewed, no changes allows you to indicate that you have reviewed the patient's histories but made no changes. History unobtainable allows you to indicate that the information is not obtainable and enter the reason in the corresponding field. Select Save & Close. Note: Each reviewed area (Med/Surg, Family, and Social Histories) contributes to the Evaluation and Management coding.

Document Historical Diagnostic Studies

You can enter historical diagnostic study information on the Diagnostic History Entry template on the Histories template. Open NextGen® Enterprise EHR. Open the Histories template. Select the Diagnostic Studies panel. Select Add. The Diagnostic History Entry template opens. Diagnostic History Entry template Select Diagnostic study type and select OK. Select Diagnostic study and select OK. The Procedure code field is filled automatically. If the diagnostic study is performed outside your practice, select Ordered elsewhere. Select Side and Site from the corresponding lists. Select Date performed to enter the date. Note: If the exact date is unknown, you can select Approximated performed date. NextGen® Enterprise EHR displays the performed or completed date, whichever is available. When both the dates are available, NextGen® Enterprise EHR displays only the completed date. Cardiology Histories Workflow

You can document the cardiology histories in the Histories - Cardiology template. The template uses standard framework functionality and is used by the nurse/technician to document a patient's history. You can document the following in the Histories – Cardiology template. Problems Medical/Surgical/Interim Diagnostic Studies Family Social Document Cardiac Problems The Problems panel enables you to document, search for, and select the chronic and secondary conditions of a patient's problems. Manage Patient's Disease You can manage the patient's disease in the Medical/Surgical/Interim panel on the Histories template. Update Past Medical History You can update the past medical history for a patient in the Edit Past Medical History template. Document the Interim History You can document all patient medical encounters, which includes but is not limited to, hospitalizations, emergency room visits, office visits, and same day surgeries. Manage Historical Diagnostic Study

To open the Diagnostic History Entry template, do one of the following: In the History Summary panel, select Diagnostic, and click Add/Edit. On the Histories template, in the Diagnostic Studies panel, click Add. The Diagnostic History Entry template opens. To update the historical diagnostic study, do the following: On the Diagnostic History Entry template, select a historical diagnostic study that need to be updated from the grid. Update the Interpretation and / or Result as needed. Click Update. The record is updated. If the record was previously added to the Reason for Visit panel, the corresponding record in also updated in the Reason for Visit panel. Close the template. To delete a historical diagnostic study, do the following: On the Diagnostic History Entry template, select a historical diagnostic study that need to be deleted from the grid. Click Delete. When you modify a historical diagnostics study on the Diagnostic History Entry template, the application automatically updates the corresponding studies reviewed record in the Reason for Visit panel. Open NextGen® Enterprise EHR. To open the Diagnostic History Entry template, in the History Summary panel, select Diagnostic, and select Add/Edit. To update the historical diagnostic study, do the following: On the Diagnostic History Entry template, select a historical diagnostic study that need to be updated from the grid. You can document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history on the Histories template.
