Document Health Promotion Plans

You can document the Health Promotion Plans such as BMI plan, depression plan, and hypertension plan of a patient. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Health Promotion Plan panel. Add a diagnosis. Select Plan and select a plan from the list.Based on the selected plan, the different sets of fields appear. To add the plan details, do the following: For BMI Plan, add the diet, physical activity, and referrals details. For Depression Plan, dd the follow-up and referrals details. For Hypertension Plan, add the diet, physical activity, referrals, and lifestyle details. Note: To turn on the Referrals section and order a referral, you must add a diagnosis. To indicate that the referral is internal, select the Internal referral checkbox. If required, in the Details field, enter any additional information. If you want to add or update patient educat

Document Pain Assessment and Plan of Care

You can document the pain assessment details and follow-up plan of care. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Pain Management panel. Note: The Pain level and Method fields display the details entered on the Vital Signs template. You cannot update the pain level and method in the Pain Management panel. Pain Management - Health Promotion Plan To manage exceptions and view measure details on the Exceptions/Measures template, select Exceptions. If details are not entered in the Pain level section on the Vital Signs template, select the following fields, and select the appropriate option in the list. Pain level Method Duration Quality To indicate to see the HPI details, select the See HPI checkbox. Enter the location of the pain of the patient. Open the Template Data Sets Setup Template

The Health Promotion Plan template is packaged with BMI Plan, Depression Plan, and Hypertension Plan. You can also create a customized health promotion plans. Open NextGen® Enterprise EHR. Select File > System \ Practice Template.The Select Template window opens. Select the Practice tab. In Show, select All. Select Data Sets Setup. In the Data Set field, select Health Promotion Plan from the list. Select Add to open the Template Data Sets Setup template, and then select Add. Place a Follow-up or Disposition Order

Open NextGen® Enterprise EHR. Do one of the following: In the Assessment Plan panel, click the A/P Details link in the left pane. On the My Plan (PT) template, click the A/P Details tab. The Assessment Plan Details template opens. Click Follow Up.The Follow-up/Disposition template opens. Follow-up/Disposition template To add or update the working assessment, select the diagnosis in the grid. Select the follow up checkbox. Enter the details and time for the follow-up visit. In the Disposition field, select an option from the list. If the applicable option is not available in the list, click the top blank row and then type a value in the Disposition field. If the patient refused the selected disposition, select the Refused checkbox. In the Reason field, select an option from the list. In the Timeframe field, select an option from the list. Click Place Order.The fields clear so that another follow-up appointment can be entered. The follow-up appointments appear in the Assessment/Plan grid

Create Custom Health Promotion Plans

You can create a custom health promotion plan. Open NextGen® Enterprise EHR. Open the Template Data Sets Setup template. In Plan Type, enter a label for the kind of plan you want. In File Maint. Dx Category, enter a disease category set up in File Maintenance. In Label for Value, enter a label that you want to search. Select the data tables and data fields that you want to retrieve values from. In Lo Value and Hi Value, enter low and high values. In Units Value, enter the type of units to display. In Other DBPicklist Name, select up to 10 custom lists. Note: The Developer, Funding Source, and Reference Updated fields support clinical decisions. When you add the configuration details in the other fields, Reference Updated is automatically filled with the current date. Select Save. Update or Remove a Health Concern

You can update or remove a health concern at any given time. If you remove a health concern, all the associated goals, interventions and outcomes are also removed. Open NextGen® Enterprise EHR. Expand the Add/Edit Health Concerns panel. From the bottom-right grid, select a health concern.The information for the health concern appears in the corresponding fields. Do one or more of the following: Modify the clinical status Modify the verification status Note: If you select the Entered-in-error verification status, the associated goals, interventions, and outcomes will be removed. Add or update comments To indicate that the health concern is resolved, select the Resolved clinical status and then enter the resolved date. Select Update. To remove a health concern, select the one to be removed.The system fills the fields above the grid with the data from the selected record.
