Add a Health Concern

Open NextGen® Enterprise EHR. From the navigation bar, select Care Plan.The Care Plan template appears. Expand the Add/Edit Health Concerns panel. Add/Edit Health Concerns panel From the Select Category grid, select a category.The Select Concern grid appears, and all patient records for the category appear in the grid. To view all the previous records, select Show all dates. If you selected Encounter Diagnosis, Social History, or Vital Signs category, you can view all the records or filter them by a date range. From the Select Concern grid, select a record.The data from the record appears in corresponding fields on the right. If a health concern is not listed in any of the categories, use the Other category to add the health concern. To modify clinical or verification statuses, select in the corresponding fields and then select a status. The default clinical and verification statuses are Active and Confirmed respectively. Add a Health Concern Using the Other Category

Open NextGen® Enterprise EHR. Expand the Add/Edit Health Concerns panel. From the Select Category grid, select Other. Manually enter the health concern or use common phrases. Select a category. To modify clinical or verification statuses, select in the corresponding fields and then select a status. The default clinical and verification statuses are Active and Confirmed respectively. Note: If you select the Entered-in-error verification status, the associated goals, interventions, and outcomes will be removed. To update the identified date, select in the corresponding field and then select a date. The default date is today's date. To enter additional information, select Comments. To enter a primary concern for the current concern, select the primary concern in the Secondary to field. Select Add.The added health concern appears in the bottom-right grid. Remove a Health Concern Goal

You can only remove a health concern goal from the encounter where the latest intervention or outcome was recorded. When you remove a goal, the system removes all associated interventions and outcomes. Open NextGen® Enterprise EHR. Open the patient encounter where the latest intervention or outcome for the goal to be removed was recorded. Open to the Goals, Interventions and Outcomes panel. Select the goal to be updated in the left grid.The system fills the fields to the right of the grid with the data from the selected record. Update a Health Concern Goal

You can update all the fields for a goal if the goal was created during the current encounter. You can update the following fields during subsequent encounters: Goal achieved Patient priority Provider priority Target Completion Comments Status Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel. Select the goal to be updated in the left grid. The system fills the fields to the right of the grid with the data from the selected record. Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. Open NextGen® Enterprise EHR. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. Remove an Intervention Outcome

You can remove an outcome only in the encounter where the latest outcome was documented for that intervention. Open NextGen® Enterprise EHR. Open the patient encounter where the latest outcome for the intervention was documented. Open to the Goals, Interventions and Outcomes panel. Select the goal you what to remove an intervention outcome.The system fills the fields to the right of the grid with the data from the selected record. Select the Intervention tab.The full Intervention tab appears with the previously entered interventions in the grid. Select the intervention with an outcome to be removed. Select the Outcomes tab.The full Outcomes tab appears with the data for the selected intervention. Select the outcome to be removed from the grid. Select Remove.The system removes the outcome.
