Add a Health Concern

Open NextGen® Enterprise EHR. From the navigation bar, select Care Plan.The Care Plan template appears. Expand the Add/Edit Health Concerns panel. Add/Edit Health Concerns panel From the Select Category grid, select a category.The Select Concern grid appears, and all patient records for the category appear in the grid. To view all the previous records, select Show all dates. If you selected Encounter Diagnosis, Social History, or Vital Signs category, you can view all the records or filter them by a date range. From the Select Concern grid, select a record.The data from the record appears in corresponding fields on the right. If a health concern is not listed in any of the categories, use the Other category to add the health concern. To modify clinical or verification statuses, select in the corresponding fields and then select a status. The default clinical and verification statuses are Active and Confirmed respectively. Update or Remove a Health Concern

You can update or remove a health concern at any given time. If you remove a health concern, all the associated goals, interventions and outcomes are also removed. Open NextGen® Enterprise EHR. Expand the Add/Edit Health Concerns panel. From the bottom-right grid, select a health concern.The information for the health concern appears in the corresponding fields. Do one or more of the following: Modify the clinical status Modify the verification status Note: If you select the Entered-in-error verification status, the associated goals, interventions, and outcomes will be removed. Add or update comments To indicate that the health concern is resolved, select the Resolved clinical status and then enter the resolved date. Select Update. To remove a health concern, select the one to be removed.The system fills the fields above the grid with the data from the selected record. Add a Patient Goal to Address a Health Concern

You can add multiple goals to a single health concern. To add multiple goals, repeat this procedure for each goal. Open NextGen® Enterprise EHR. Expand the Goals, Interventions and Outcomes panel. The Select a Health concern to document section appears with all previously added health concerns. Goals, Interventions and Outcomes panel Select a health concern from the list. The information about the selected health concern goal fields appears on the right. Goal details on the Goals, Interventions and Outcomes panel In the Goal field, type a goal description to address the health concern, or use Common Phrases. Indicate if the goal is a patient goal or a provider goal. Verify that the life cycle status of the goal is set to Active. You can also change the status. Record an Outcome for an Intervention

Open NextGen® Enterprise EHR. Open to the Goals, Interventions and Outcomes panel. The Select a Health concern to document grid is populated with all previously identified health concerns. Select a health concern from the list. The selected health concern appears in the Health Concern field and the data entry fields for goals appear to the right. Select the goal that you are adding an intervention outcome for. The data for the selected goal fills the data entry fields to the right. Select the Interventions tab. The full Interventions tab appears with the previously entered interventions in the Select an Intervention to document grid. Select the intervention in the grid that you are reporting an outcome for. Select the Outcomes tab. The full Outcomes tab appears with the data entry fields for the outcome to the right. Create an Intervention to Achieve a Goal

Create a Care Plan

Before you begin Make sure that the existing care plans for the patient are in Completed status. Open NextGen® Enterprise EHR. From the navigation, select Care Plan.The Care Plan template appears. Care Plan template Select Create a New Care Plan.All the panels gets activated. Activated panels on the Care Plan template Select a status, and an intent.The default values are Active, and Plan respectively. Add a health concern. Add goals to address health concern. Add interventions to achieve the goals. Record functional and cognitive assessments. Select Save and Close. Note: To edit a completed care plan, you must update the status of the current plan to Entered-in-Error, which removes the current care plan's data and redirects you to the previous care plan.
