Enter the Medication Allergy Details
- After you selected an Allergen and Allergy Type, select a Location and Provider.
 - Select the Intolerance checkbox if the patient is intolerant to the medication.
   
 - To specify the severity of the allergy, select a Severity from the list.
 - To specify that the allergy you are adding is recorded elsewhere, select the Recorded Elsewhere  checkbox.
   The Source field is available to enter a source.
 - To enter the date the allergy or its symptoms began, select the Onset/Sym Search button. A calendar opens to select an onset date.
 - To enter the date the allergy was resolved, select the Resolved Search  button. A calendar opens to select a resolved date.
   Note: If the Onset Date or Resolved date is unknown, you can use 00/00/0000.
 - To specify the type of reaction the patient has due to the medication allergy, select the Reaction arrow and select a reaction type from the list.
 - Verify the Allergy Type field specified in the Allergy Selection window.
 - Enter additional comments in the Allergy Comments field.
 - To save the medication allergy, select Add.
   Note: Adding a new allergy to the patient chart automatically triggers the Drug-Allergy Interactions Check. If an interaction exists, a warning window appears. The display of allergy interaction is based on the display levels set for the system and/or a specific user.
 - Select Clear to reset the data fields to their default values to add another allergy.
