Document the Education History for an Adult Client
In the Education - Adult panel, under Education History, select No or Yes for Special needs. To document additional information, click the If Yes, comment field. Education-Adult Enter the client's Education, Type of school and highest Grade in school. Record the client's Major and select a Degree obtained from the associated list. To document vocational training, click Vocational training and indicate if the training was Completed. To indicate current enrollment, select If not completed, are you currently enrolled and enter additional details in the field that follows. Document Social Supports
On the Social Supports panel of the BH CA Social History template, enter the client's Friendships/social/pets/peer support relationships. Include data about the client's community involvement, volunteer activities, leisure/recreation, and other interests in the Meaningful activities field. Click the Religion/spirituality field to open a list of religion choices. To document additional information, click Religion/spirituality comments. Enter one or more values to record the client's Race and if necessary, add Race comments.If this information was entered in the NextGen® Enterprise PM application, it populates here. Document the client's involvement in Community supports/self-help groups. Click the Ethnicity field to launch a new widget that enables you to select multiple ethnicity for the patient.If this information was entered in the NextGen® Enterprise PM application, it populates here. Enter Developmental History Information
All of the data in the Development History panel carry forward data from encounter to encounter for ease of documentation. Navigate to the Development History panel. Select from one of the links at the top of the panel: Developmental Milestones - document Child Developmental Record (data shares from the Pediatrics template set) and/or Child Development History (information can be free-typed). Document Child Abuse/Neglect
On the BH _ Brief Assessment template, navigate to the History Summary panel and click the Confidential link. The Peds Confidential History - Substances template opens. Click the Child Abuse link in the left pane.The Peds Confidential History - Abuse template opens. Click the Historian field to select a description of the person providing the information. Under Child Abuse History, select the applicable option for Child Protective Services involved. If Yes, in the Case worker field, enter the case worker's name, Phone, and select one of the following options for Child protective services determination: Pending (as of today's date) Abuse determined (founded, indicated, substantiated) Unsubstantiated Unknown Select any of the following options for Type of abuse: Physical Sexual Verbal Psychiatric/Emotional Medical Neglect Exposure to domestic violence Other In the Offender 1 and Offender 2 fields, click to enter the name. Add Education and Leisure Activities Details
You can document the details of education and leisure activities of a patient on the Pediatric Social History – Education template. Open NextGen® Enterprise EHR. To open the Pediatric Social History – Education template, do one of the following: On the Histories template, expand the Social panel, select Education, and then click Add/Edit. In the History Summary panel of the Intake template, click Social, click Education, and then click Add/Edit. Pediatric Social History – Education In the Historian field, enter the relationship of the person who provides the patient information. In the Education section, do the following: Enter the patient's current school name and grade. Indicate if the patient has repeated any grades in school. If you select Yes, then enter the grade and the reason for the repetition. Select the grades the patient received in school. Document Counseling and Education Details
You can document patient's counseling and education details using the Counseling Details template. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template.The Care Guidelines Paged View template opens. In the Care Guidelines panel, select the Grid View link.The Care Guidelines grid view template opens. Expand the Nutrition panel, select the Details link.The Counseling Details template opens.
