On the BH _ Brief Assessment template, navigate to the History Summary panel and click the Confidential link.
The Peds Confidential History - Substances template opens.
Click the Child Abuse link in the left pane.
The Peds Confidential History - Abuse template opens.
Click the Historian field to select a description of the person providing the information.
Under Child Abuse History, select the applicable option for Child Protective Services involved.
If Yes, in the Case worker field, enter the case worker's name, Phone, and select one of the following options for Child protective services determination:
Select any of the following options for Type of abuse:
Physical
Sexual
Verbal
Psychiatric/Emotional
Medical Neglect
Exposure to domestic violence
Other
In the Offender 1 and Offender 2 fields, click to enter the name.
Select Yes or N for Offender has current contact with child.
Enter start and stop dates in the fields next to the Date alleged abuse began/stopped label or if unknown, select the Exact date unknown option and select the appropriate value from the list.
Enter the date the child's issue was reported to Child Protective Services or if not known, click the Exact date unknown option and select a value from the list.
Select Yes or No for the Patient has developmental delay or other communication/learning barrier.
If you select Yes, type details in the Specify field.
Next to Patient has mental health history, select No or Yes.
If Yes, enter details in the Specify field.
In the Details of reported abuse/assault section, use the Historian 1 and Historian 2 fields to enter the relationship of the person reporting the information and enter the information from the historians in the corresponding fields.
In the Comments field, enter any additional details.
Click Add to display the documented details in the grid.
For easier reading, click the ExpandedView link to open the Peds Confidential Hx -Abuse (expanded) template, which provides a larger view of the data grid.
Click Save & Close to return to the Histories panel.
