Record Confidential Pediatric Placement History
On the BH _ Brief Assessment template, navigate to the History Summary panel and click the Confidential link. The Peds Confidential History - Substances template opens. Click Psychiatric/Placement in the left pane. The Peds Confidential History - Psychiatric template opens. Under Placement History, select No or Yes for Currently placed out of home. If the Yes option is selected, enter the Date of current placement, Placement type, Home type, and Placement status. Select the Yes or No option for Parental rights have been terminated. Enter the Residential caregiver name and the Next court date. Indicate if there is a Court document on file and if Yes, enter the Date of document and Jurisdiction. Document Visitation restrictions and additional details in the Explain field, as needed. Record if the child is Receiving community health services and if Yes, enter the Agency and Phone number Document Other services being implemented and click Add to enter the documented data in the grid. Document Child Abuse/Neglect
On the BH _ Brief Assessment template, navigate to the History Summary panel and click the Confidential link. The Peds Confidential History - Substances template opens. Click the Child Abuse link in the left pane.The Peds Confidential History - Abuse template opens. Click the Historian field to select a description of the person providing the information. Under Child Abuse History, select the applicable option for Child Protective Services involved. If Yes, in the Case worker field, enter the case worker's name, Phone, and select one of the following options for Child protective services determination: Pending (as of today's date) Abuse determined (founded, indicated, substantiated) Unsubstantiated Unknown Select any of the following options for Type of abuse: Physical Sexual Verbal Psychiatric/Emotional Medical Neglect Exposure to domestic violence Other In the Offender 1 and Offender 2 fields, click to enter the name. Document Pediatric Confidential Incarceration History
On the BH _ Brief Assessment template, navigate to the History Summary panel and click the Confidential link. The Peds Confidential History - Substances template opens. Click Psychiatric/Placement in the left pane. The Peds Confidential History - Psychiatric template displays. In the Incarceration History section, select the Yes or No option for History of incarceration. If Yes, enter the appropriate details in the additional fields: Crime Treatment status Duration of incarceration Facility Duration of treatment Duration of probation Status Click Add to add the data in the grid. Document Pediatric Confidential Psychiatric History
On the BH _ Brief Assessment template, navigate to the History Summary panel and click the Confidential link. The Peds Confidential History - Substances template opens. Click Psychiatric/Placement in the left pane. The Peds Confidential History - Psychiatric template displays. Click the Historian field to select the relationship of the person providing the details. In the Psychiatric History section, select the No or Yes option for History of suicidal thoughts, History of homicidal thoughts, and Diagnosed with psychiatric problem. Enter the treating Psychiatrist's name and Phone number in the corresponding fields. Select the No or Yes option for the Signed consent for communication with PCP. Enter Therapist name and Phone number. Select No or Yes for the Signed consent for communication with PCP. Pediatric Confidential History
Use the Peds Confidential History template set to document a pediatric patient’s private information. Links are available in the left pane to document substance use, psychiatric/placement, sexual practice, and child abuse history. Note: The patient must meet the practice's set parameters for the Pediatric Age on the Practice Configuration template in order to use the Peds Confidential History templates. Document Pediatric Drug History
You can document the drug history of a pediatric patient. Open NextGen® Enterprise EHR. To open the Peds Confidential History - Substances template, do one of the following: In the Social panel on the Histories template, click the Confidential History link. In the History Summary panel on the Intake template, click the Confidential link. In the Drug Use/Abuse section, indicate if the patient uses drugs. If you select Yes or Formerly, then do the following: Document the drug type, frequency, and the route of taking drugs. To indicate that the patient has quit taking a drug, select the Quit checkbox near the respective drug. Indicate if the patient has sought treatment for drug abuse. If you select Yes, then enter the number of times the patient was treated and the date of the last drug treatment, then select the Outpatient or Inpatient checkbox to document the type of treatment.
