Document Military Service
On the Military Service panel, select No or Yes to the question, Have you ever served in the military. Select No or Yes for Is someone in your family, or a significant other, in the military. Enter Education Information for a Child/Adolescent
In the Education - Child/Adolescent panel, select No or Yes for Regular education classroom. Type the child's Name of school and Current grade. The Education classification and Other sections provide the ability to make multiple selections. Under Additional information, document No or Yes if the client is involved in a: Current Individual Education Plan Current 504 Plan To provide a brief overview of the client's academic functioning, click the Comments on past and current academic functioning field. In the Test or other evaluation results section, select No test results were reported or type test or evaluation results in the Comments field. In the Attendance section, either select Not a problem or enter details in the Comments field. In the Previous grade retention and Suspension/expulsion sections, select Denied or type an entry in the respective Comments field. Enter any Additional barriers to learning. Document Social Supports
On the Social Supports panel of the BH CA Social History template, enter the client's Friendships/social/pets/peer support relationships. Include data about the client's community involvement, volunteer activities, leisure/recreation, and other interests in the Meaningful activities field. Click the Religion/spirituality field to open a list of religion choices. To document additional information, click Religion/spirituality comments. Enter one or more values to record the client's Race and if necessary, add Race comments.If this information was entered in the NextGen® Enterprise PM application, it populates here. Document the client's involvement in Community supports/self-help groups. Click the Ethnicity field to launch a new widget that enables you to select multiple ethnicity for the patient.If this information was entered in the NextGen® Enterprise PM application, it populates here. Record the Client's Legal Issues
Open the BH CA Social History template and expand the Legal panel. Under Status, select No or Yes for the following questions: Does individual served have a legal guardian, representative payee or conservatorship Is there a need for a legal guardian, representative payee, conservatorship or special needs trust - Use the If yes, explain field to enter more details. Select No or Yes for Does the individual have a history of, or current involvement with the legal system. To enter comments about the client's involvement in the legal system, click Comments. Document the Patient's Employment Status
You can document the patient's employment status for Behavioral Health by using the BH CA Social History template. Open NextGen® Enterprise EHR. Open the Intake template, and then select the Behavioral Health specialty and Onsite Consult visit type. In the navigation bar, select the History - BH link.The BH CA Social History template opens. Expand the Employment Status panel. In Current employment status, enter the average number of hours worked per week. Optional: If not currently employed, enter the reason. If the employment belongs to Other, then enter the details. Select No, Yes, or Uncertain as applicable. In Comments, enter any additional information about the client's work status. Maintain a Record of the Client's Trauma History
The data in this panel copies forward from previous encounters and shares with the same panel on the Brief Assessment template. Expand the Trauma History panel. Trauma History If there is no history of trauma, select None reported to set the options below for the different types of experiences to be unavailable. Document the client's history and current experience of trauma in the Does individual report a history of, or current experience of section If Other is selected, an explanation or description can be typed into the adjacent field.
