Document Sexual Issues

The field data on the Sexual Issues panel carries forward from encounter to encounter. Open the BH CA Social History template and expand the Sexual Issues panel. Enter the client's Sexual Preference. Document Social Supports

On the Social Supports panel of the BH CA Social History template, enter the client's Friendships/social/pets/peer support relationships. Include data about the client's community involvement, volunteer activities, leisure/recreation, and other interests in the Meaningful activities field. Click the Religion/spirituality field to open a list of religion choices. To document additional information, click Religion/spirituality comments. Enter one or more values to record the client's Race and if necessary, add Race comments.If this information was entered in the NextGen® Enterprise PM application, it populates here. Document the client's involvement in Community supports/self-help groups. Click the Ethnicity field to launch a new widget that enables you to select multiple ethnicity for the patient.If this information was entered in the NextGen® Enterprise PM application, it populates here. Document the Client's Living Situation

The Living Situation panel on the BH CA Social History template enables you to document specific details about the client's housing situation. Open the BH CA Social History template and expand the Living Situation panel. Record Where is the individual currently living. Note: The list for living situation may be edited by the practice. Enter the Residential care/treatment facility details. Indicate if the client is At risk of losing current housing and enter additional Comments. Answer No or Yes to the question: Satisfied with current living situation. Click Add next to the Residence field to enter who is living with the client. To enter additional information related to the living situation, click Comments.Notice the character counter at the bottom of the Comments field. This lets the user know how many of the 500 available characters are left to ensure that all of the information prints on the generated document. Document the Patient's Employment Status

You can document the patient's employment status for Behavioral Health by using the BH CA Social History template. Open NextGen® Enterprise EHR. Open the Intake template, and then select the Behavioral Health specialty and Onsite Consult visit type. In the navigation bar, select the History - BH link.The BH CA Social History template opens. Expand the Employment Status panel. In Current employment status, enter the average number of hours worked per week. Optional: If not currently employed, enter the reason. If the employment belongs to Other, then enter the details. Select No, Yes, or Uncertain as applicable. In Comments, enter any additional information about the client's work status. Maintain a Record of the Client's Trauma History

The data in this panel copies forward from previous encounters and shares with the same panel on the Brief Assessment template. Expand the Trauma History panel. Trauma History If there is no history of trauma, select None reported to set the options below for the different types of experiences to be unavailable. Document the client's history and current experience of trauma in the Does individual report a history of, or current experience of section If Other is selected, an explanation or description can be typed into the adjacent field. Maintain Family Data for the Client

On the Family Information panel of the BH CA Social History template, select No or Yes for Does the patient have children. If Yes, enter demographic information about the client’s children. To delete data from the related fields before it is saved to the grid, click Clear. Click Add to enter the information into the grid. Document Family history and relationships. The Family health history section is available for medical history documentation. Click Add to enter new data on the Family Health History template. Click the History Review link to launch the template on which you can determine what prints in the generated documentation. Record the Pertinent family medical, mental health and substance use history including Adult Child of Alcoholic (ACOA)/Child of Substance Abuser (COSA) status.
