Add a Patient's Previous Address

You can add a previous address for a patient. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Address panel. Address In the Address History section, enter the start and end dates to indicate the duration of residence at this address. Note: By default, the Type field is historical. Enter the patient's previous address and phone number. Enter the ZIP code. The city, state, county, and country information fields are automatically filled. Note: You can also edit or manually enter the information. Select Add. The previous address is added to the grid in the Address panel. Parent topic: Managing the Telephone Call Template

Open the Patient Demographics Template

You can document patient details such as contact number, address, Primary Care Physician (PCP), primary practice provider, insurance, pharmacy, provider, emergency contact, employment, and reminder details, on the Patient Demographics template. Open NextGen® Enterprise EHR. Select a patient record. To open the Patient Demographics template, do one of the following: Open the Templates module, and double-click Patient Demographics in the Demographics tab. On the Home Page template, select the Demographics link. The Patient Demographics template opens. Add Patient Demographic Information

You can add demographic details of a patient on the Patient Demographics template. Open NextGen® Enterprise EHR. Open the Patient Demographics template, and then expand the Patient Information panel. Patient Information Enter values for the following fields: State of birth Country of birth Blood type Enter information in the Race and Ethnicity fields. Note: You can add multiple values for race and ethnicity. To enter the patient's social security number, preferred language, and marital status on the Modify Patient Information template, select the corresponding fields.The Modify Patient Information window opens. The social security, preferred language, and marital status information you update in the Modify Patient Information window automatically appears in the Patient Information panel of the Patient Demographics template. The Telephone Call template and the Telephone Communication template enable you to record information gathered during a telephone conversation. You can use the Call/Communication sub-navigational link to open the Telephone Call template. View an Updated Address for a Person or Patient

When an address is automatically updated from the Change of Address Update process, a message is recorded in the demographic history for the person or patient. From the Lookup button on the main toolbar, search and select the desired person or patient. The Modify Person Information or Update Patient Information window opens. The current address displays in the Demographics > Address section. From the navigation pane on the left, click History. If the address was updated from the Change of Address Update process, an entry is listed in the Person History section with the following information: Date/Time: The date and time the update process was run. Creator: The name of the user who ran the update process. Description: Address Changed Via "Change of Address Update" Process. Old Information: The address prior to the update process. Document the Patient's Location and Contact Information

You can document the patient's basic information such as the name, address, and contact information. Open the Demographics (BH) template. Enter information in the Patient Information, Contact Information, and Address History panels. Enter patient's contact details in the corresponding fields. Select Add.
