Create a Treatment Plan for the Patient

Treatment plans are set up on the PT Treatment Setup template. The PT Treatment Setup template displays the Menu and the Treatment Plan panels. The Menu panel displays a list of treatment categories in the left pane. The Treatment Plan panel grid displays the treatment options previously selected for the patient. Treatment plans configured on the PT Treatment Setup template appear in the Performed Today section in the Treatment Panel and carry forward to future encounters. Open NextGen® Enterprise EHR. On the Objective template, click the Treatment button in the Treatment panel to launch the PT Treatment Setup template. Pt Treatment Setup Click the Body region field and select from the list.The Menu panel grid displays all treatment options for the selected region. Select a treatment category from the left pane.The Menu panel grid refreshes to display that category’s treatment options for the region. Physical Therapy Treatment Panel

The Treatment panel enables you to document treatments performed during an encounter. Information documented here is also used for billing purposes. The Treatment grid displays details about the treatment plans. The fields in the Performed Today section below the grid function as a worksheet for documentation of treatment activities performed. Note: Only those treatments detailed in a row with the Performed checkbox selected are billed. Treatment Section

The treatment grid shows therapies and education that you have documented for the patient. You can document therapies completed with the patient using any of the treatment templates in the left-hand list. The therapies documented on the templates are billable, with the exception of the Education template, which always shows a billable Total Time entry of zero. Enter or Review Surgical Data, Authorized Visits, and Diagnosis

The General panel enables you to enter or review surgical data, authorized visits and diagnosis. Most of the information that you document in the General panel are one-time entries. Open NextGen® Enterprise EHR. From the Subjective tab, expand the General panel, and then select one of the patient type options, such as Established patient or New patient. General Note: The Established patient option is automatically selected if a previous encounter exists for the patient. To select the relationship of the person providing the details for the patient, click the Historian field. Enter the Onset date and the Start of care date. Note: The Referring provider field displays the provider name entered at the encounter level. Select the surgical procedure to be performed. Select No or Yes for Patient awareness of diagnosis and prognosis. Complete Billing for Procedure Items

Open NextGen® Enterprise EHR. In the Assessment panel on the PT Finalize template, select an assessment listed in the Encounter Diagnosis grid. To associate the diagnosis with a treatment, click the first available Dx code field in the row displaying the treatment.All Procedure rows displaying treatments that were performed automatically display the selected encounter diagnosis code under the Dx code field. This is done to eliminate the need to keep re-selecting the assessment and clicking in the Dx code field for the following treatment. Up to four diagnosis codes can be associated with a treatment procedure. If appropriate, click the treatment procedure’s first available Modifier field and select the modifier from the picklist.Up to three modifiers can be entered. To enter a modifier code that is not listed, select the top blank line and type the desired value. To suppress billing for a treatment procedure, select the corresponding Suppress billing checkbox. Document Therapy and Treatment Orders

The Therapy Orders template enables you to enter detailed information about prescribed therapy and treatment orders for the patient including the diagnosis, treatment type, physician goals and the number of authorized visits. Open NextGen® Enterprise EHR. To open the Therapy Order template, do one of the following: In the navigation bar, select Orders. In the Assessment / Plan panel on the SOAP template, select Therapy Orders. Select a diagnosis or an assessment in the Encounter Diagnoses or Treating Assessment section. Note: You can add or update the records in the Encounter Diagnoses or Treatment Assessment grids, if required. When you select a diagnosis in the Encounter Diagnoses or Treating Assessment grid, the diagnosis details appear in the corresponding read-only fields. You can select up to 4 diagnoses.
