Document Treatments Performed Today

The Performed Today section is used to document what treatments performed during the treatment encounter and enter additional information. Treatments, CPT and Time (mins) values carry forward to future encounters where they can be modified or updated as needed. Treatment Open NextGen® Enterprise EHR. On the Objective template, navigate to the Treatment panel. In the Performed Today section, complete treatment details performed. Click the Weight field and use the numeric keypad to enter the weight used when performing the exercise/treatment, if applicable. Click the Sets field to enter the number of sets of the exercise/treatment performed, if applicable. Click the Reps field to enter the number of repetitions performed in each set of the exercise/treatment. Click the Exertion field and select the exertion level when the exercise/treatment was being performed from the picklist. Document the Treatment Plan on My Plan

The treatment plan for the patient can be documented on the My Plan (PT) template that enables providers to place multiple orders from a single template. The My Plan (PT) template is based on order sets for diagnosis categories that can be modified and managed by clients using practice templates. The My Plan (PT) template appears within NextGen® Adaptive Content Engine by default and can be easily replaced with the My Custom Plan template after a simple configuration in the Practice Configuration practice template. The My Custom Plan template can be personalized to suit your practice's specific needs and is less click-intensive. Enter New Therapy Goals for the Patient

Therapy goals are based on information documented for the reason for visit, History of Present Illness (HPI), and physical exam for the patient. Preconfigured therapy goals configurations are packaged with the system. Clients can add customized goals and modify or remove existing ones. Goals can be added, updated or removed. Open NextGen® Enterprise EHR. On the Objective template, on the Goals panel, click the Add button to launch the Therapy Goals template, which displays accumulated goals listed in the Available Goals grid. Therapy Goals Highlight a row in the Available Goals grid to select a goal. Click the Goal type field and select the type of goal from the picklist.The remaining fields display additional details for the goal. Enter or Review Surgical Data, Authorized Visits, and Diagnosis

The General panel enables you to enter or review surgical data, authorized visits and diagnosis. Most of the information that you document in the General panel are one-time entries. Open NextGen® Enterprise EHR. From the Subjective tab, expand the General panel, and then select one of the patient type options, such as Established patient or New patient. General Note: The Established patient option is automatically selected if a previous encounter exists for the patient. To select the relationship of the person providing the details for the patient, click the Historian field. Enter the Onset date and the Start of care date. Note: The Referring provider field displays the provider name entered at the encounter level. Select the surgical procedure to be performed. Select No or Yes for Patient awareness of diagnosis and prognosis. Configure Physical Therapy Goals

Open NextGen® Enterprise EHR. From the File menu, select System/Practice Template. Select the Practice tab. Set Show to All. Scroll down and double-click PT Goal Administration. The PT Goal Administration template opens. Goal Administration Select the Template name field, then select the name of an HPI or PE template from the list. Select the Table field, then select the name of a table from the list. Select the Field field, then select the name of the field from the list. Select the Goal type field, then select the type of goal from the picklist. Select the Custom description field to type in a description. Select the Value mask field to type in a numeric value. Select the Value translation field to enter an interpretation for the value entered in the Value mask field. Select the fields for Normal values and enter one or more normal values for the goal.By default, the Normal values fields display ‘NULL’.
