Pediatrics Well Child Reasons for Visit

The Well Child visit in Pediatrics enables you to document age-specific complaints for the following reasons for visit: Well Child birth to 1 Year Well Child 1-4 Years Well Child 5-10 Years Well Child 11-21 Years Note: This topic provides information on only those reasons for visit that opens dynamic HPI template associated with Pediatrics, Well Child visit. Treatment visits can be based on the non-laterality view as most patients do not have an issue that pertains to one side of the body or the other. Document the Well Child Reason on the Generic HPI Template

You can document the reason for well child visit on the generic HPI template. Open NextGen® Enterprise EHR. Open the Pediatric SOAP template. In the Reason for Visit panel, click the Well child reason in the Reason for Visit grid. The Generic Free Form1 - HPI template opens. Generic Free Form1 - HPI Enter the reason for visit details in the applicable fields. Click Save & Close. The documented data appears in the grid. Note: The E & M coding information icon on the generic HPI template opens the Generic Free Form Information template which provides details on the E&M coding points. However, E&M coding points are not assigned for a well child visit. View Orders and Lab Results You can view all the orders and lab results of a well child visit. Reason for Visit for Well Child

You can use the Reason for Visit panel on the Pediatric SOAP template to review and update the reasons for visit added during the intake process. The Well child HPI appears on the grid automatically if you select Well child for Visit Type. Reason for Visit In addition to documenting the specialty specific reason for visit available in the left pane, you can also document a well child visit on the a generic HPI template by selecting Well child or Well child HPI in the Reason for Visit panel. In the Reason for Visit panel, you can: Review and update the social history of the patient using the Pediatric Social History link. Open the Interval History- Nutrition template using the Nutrition/Elimination link. Document substance use, psychiatric, placement, sexual practices, and child abuse history of the patient using the Confidential History link. The dynamic HPI templates for the Pediatrics Well Child visit type have both generic and distinct sections available for documentation. Generic sections are common to all reasons for visit and distinct sections are specific to each reason for visit. Combinations of these sections enable the complete documentation of the patient's reason for visiting the office during the current encounter. The following table defines the distinct and generic sections that appear for all Pediatrics, well-child reasons for a visit. Access Well Child Dynamic HPI

In the Intake template, select the Specialty as Pediatrics and Visit Type as Well Child. Click the Pediatric SOAP tab. In the Reason for Visit panel, select well child hpi from the left pane. General Information for Well Child Visit

You can enter a patient's general information. The General panel on the Pediatric SOAP template displays whether the patient is new or established. You can view the patient's well child age and review the previous well child visits and documents. General panel In the General panel, you can do the following: View the patient's well child age based on the patient's date of birth in the Well Child Age list, and select a different well child age from the Select different well child visit list, if required. When you select a different well child age in Select different well child visit, the selected age appears in Well Child Age. View the patient's most recent well child encounter in the Previous Well Child Visit field. View the Chart Note or Master document from previous encounters using the Open Selected Well Child Visit button. If the previous encounter selected in Previous Well Child Visit does not have an associated Chart Note or Master document, the Open Selected Well Child Visit butt
