Assessment and Plan for Well Child

You can use the Assessment / Plan panel on the Pediatric SOAP template to document assessments, postoperative concerns, follow-up details, treatment plans, lab orders, referrals, office procedures, anticipatory guidance, and more. In the Assessment / Plan panel on the Pediatric SOAP template, you can find the following options for a well child encounter. The Anticipatory Guidance button to document the anticipatory guidance details. The assessment Encounter for routine child health examination without abnormal findings (Z00.129) in the grid to document the routine child health exam. You can click the assessment to directly enter the assessment and plan details on the Add or Update Assessment template. You can also use the Add Assessment button on the Add or Update Assessment template to enter assessment and plan details. Pediatric Screening for Well Child

You can document and manage the screening tests and results in the Screening panel. In the Screening panel on the Pediatric SOAP template, you can do the following: Document different screenings for a patient using the screening buttons in the panel. View the patient's screening tests and results from the grid. Remove some of the screening results from the grid. Open the document attached to the screening results, if available. View the lab results for the pediatric patient. General Information for Well Child Visit

You can enter a patient's general information. The General panel on the Pediatric SOAP template displays whether the patient is new or established. You can view the patient's well child age and review the previous well child visits and documents. General panel In the General panel, you can do the following: View the patient's well child age based on the patient's date of birth in the Well Child Age list, and select a different well child age from the Select different well child visit list, if required. When you select a different well child age in Select different well child visit, the selected age appears in Well Child Age. View the patient's most recent well child encounter in the Previous Well Child Visit field. View the Chart Note or Master document from previous encounters using the Open Selected Well Child Visit button. If the previous encounter selected in Previous Well Child Visit does not have an associated Chart Note or Master document, the Open Selected Well Child Visit butt

Physical Exam for Well Child

You can examine a patient and document the information in the Physical Exam panel on the Pediatric SOAP template. In the Physical Exam panel, you can do the following: Use the One Page Exam link to document a complete exam on a single template or click the single system exams from the left pane and its submenu. When you click the single system exam or its submenu, it opens the respective template. Select a system from the list if the applicable system does not appear in the left pane using the Additional link. The physical exam template associated with the selected system appears to document the appropriate physical exam findings. Use the Office Diagnostics button to document or review diagnostics. Document the Well Child Encounter Dynamically

You can dynamically document the reason for visit of a well child using the Well child HPI in the Reason for Visit panel. If the intake staff selects the Well child HPI reason during the intake process, the Well child HPI reason automatically appears on the Reason for Visit panel. If the intake staff does not select the Well child HPI reason, you can select the Well child HPI reason from the left pane. Open NextGen® Enterprise EHR. Open the Pediatric SOAP template. In the Reason for Visit panel, click Well child HPI.The HPI template opens. HPI Note: By default, the application selects the well child age group in the Reason field based on the patient's date of birth. To change the well child age group, click the list in the Reason field and select a different well child age group. Document the Well Child Reason on the Generic HPI Template

You can document the reason for well child visit on the generic HPI template. Open NextGen® Enterprise EHR. Open the Pediatric SOAP template. In the Reason for Visit panel, click the Well child reason in the Reason for Visit grid. The Generic Free Form1 - HPI template opens. Generic Free Form1 - HPI Enter the reason for visit details in the applicable fields. Click Save & Close. The documented data appears in the grid. Note: The E & M coding information icon on the generic HPI template opens the Generic Free Form Information template which provides details on the E&M coding points. However, E&M coding points are not assigned for a well child visit. View Orders and Lab Results You can view all the orders and lab results of a well child visit.
