Document the Well Child Encounter Dynamically

You can dynamically document the reason for visit of a well child using the Well child HPI in the Reason for Visit panel. If the intake staff selects the Well child HPI reason during the intake process, the Well child HPI reason automatically appears on the Reason for Visit panel. If the intake staff does not select the Well child HPI reason, you can select the Well child HPI reason from the left pane. Open NextGen® Enterprise EHR. Open the Pediatric SOAP template. In the Reason for Visit panel, click Well child HPI.The HPI template opens. HPI Note: By default, the application selects the well child age group in the Reason field based on the patient's date of birth. To change the well child age group, click the list in the Reason field and select a different well child age group. View Orders and Lab Results

You can view all the orders and lab results of a well child visit. Open NextGen® Enterprise EHR. Open the Pediatric SOAP template. In the Reason for Visit panel, click the Well child reason in the Reason for Visit grid. The Generic Free Form1 - HPI template opens. Click the Review of Test(s) link, and select a Category to view the orders and lab results of the patient. All the orders and lab results of the selected category appear in the grid. Figure 1. Reason for Visit for Well Child

You can use the Reason for Visit panel on the Pediatric SOAP template to review and update the reasons for visit added during the intake process. The Well child HPI appears on the grid automatically if you select Well child for Visit Type. Reason for Visit In addition to documenting the specialty specific reason for visit available in the left pane, you can also document a well child visit on the a generic HPI template by selecting Well child or Well child HPI in the Reason for Visit panel. In the Reason for Visit panel, you can: Review and update the social history of the patient using the Pediatric Social History link. Open the Interval History- Nutrition template using the Nutrition/Elimination link. Document substance use, psychiatric, placement, sexual practices, and child abuse history of the patient using the Confidential History link. Document the Patient's Reason for Visit as a Provider

The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. Edit a Reason for Visit on the SOAP Template

You can edit existing reasons for visits on the SOAP template. After the intake staff documents the patient's reasons for visit, you can edit the reasons for visit on the SOAP template to document additional details. Open NextGen® Enterprise EHR. On the SOAP template, expand the Reason for Visit panel. To edit a reason for visit, do the following: Select the HPI that you want to edit in the Reason for Visit grid. The corresponding HPI template opens. Edit the details in the HPI template. Select Save & Close. Access Well Child Dynamic HPI

In the Intake template, select the Specialty as Pediatrics and Visit Type as Well Child. Click the Pediatric SOAP tab. In the Reason for Visit panel, select well child hpi from the left pane.
