Document the Well Child Encounter Dynamically

You can dynamically document the reason for visit of a well child using the Well child HPI in the Reason for Visit panel. If the intake staff selects the Well child HPI reason during the intake process, the Well child HPI reason automatically appears on the Reason for Visit panel. If the intake staff does not select the Well child HPI reason, you can select the Well child HPI reason from the left pane. Open NextGen® Enterprise EHR. Open the Pediatric SOAP template. In the Reason for Visit panel, click Well child HPI.The HPI template opens. HPI Note: By default, the application selects the well child age group in the Reason field based on the patient's date of birth. To change the well child age group, click the list in the Reason field and select a different well child age group. Ambulatory Surgery Reason for Visit

In Ambulatory Surgery Center (ASC), the procedures to be performed on the patient form the reasons for visit. In the Ambulatory Surgery specialty, the procedures documented in the framework templates are used as the reasons for visit that are linked to the dynamic HPI template. The procedure details documented in the Procedure panel appear in the Reason for Visit panel on the ASC - Physician Procedure template. The procedure(s) in the Reason for Visit panel grid opens the dynamic HPI template to add or update additional details on the procedure. Parameters of Ambulatory Surgery The Procedure and Indications parameters of the ambulatory surgery specialty are associated with the findings to document the procedure details. Pediatric Sub-Specialty Reasons for Visit

The Pediatrics sub-specialty Reason for Visit panel includes a list of chief complaints that are configured to launch the following dynamic HPI templates: Autism Chest pain Fibromyalgia Joint pain Juvenile Idiopathic Arthritis Med check Palpitations Scleroderma Note: Not all of the reasons for visit associated to Pediatrics use a dynamic HPI template. Physical Therapy Reasons for Visit

In the Physical Therapy template set, certain reasons for visit use the dynamic HPI template Reasons for Visit. The dynamic HPI view and the laterality or non-laterality view that appears is based on the particular History of Present Illness (HPI) parameter being documented. Physical Therapy HPI parameters that involve location for a chief complaint related to extremities (for example, pain or numbness related to a hip or knee) appear in the laterality view. Parameters that refer to the chief complaint overall, such as status or place of injury, appear in the non-laterality view. Reasons for visit that do not involve laterality, such as lumbar spine, use the non-laterality view for all HPI parameters. Treatment visits can be based on the laterality or non-laterality view based on the selected reason for visit and the HPI element being documented. Orthopedic Reasons for Visit

The dynamic HPI templates for the Orthopedic specialty are based on the laterality and non-laterality views. Pediatrics Well Child Dynamic Parameters

The dynamic HPI templates for the Pediatrics Well Child visit type have both generic and distinct sections available for documentation. Generic sections are common to all reasons for visit and distinct sections are specific to each reason for visit. Combinations of these sections enable the complete documentation of the patient's reason for visiting the office during the current encounter.
