Laterality View of Dynamic HPI

The documentation of limbs on one side of the body or the other is accomplished in the laterality view on the dynamic History of Present Illness (HPI) template. This view uses two separate grids. Non-Laterality View of Dynamic HPI

The non-laterality view contains a single grid and a field that fills with the concatenated text. This text appears in the Reason for Visit grid and on the generated documents. When you click Intro, the options available for the introductory text varies based on the chosen Reason. In addition, the sections available in the left pane are different for each Reason. These sections of the HPI enable you to dynamically build the historical note. An asterisk at the beginning of the section indicates that it is added to the concatenation. The selected data also populates in the History of Present Illness column on the shared grid that appears in both the HPI and SOAP templates. The concatenation functionality is set up so that you can skip sections in the left pane that are not needed. There are no rules pertaining to the order of selection or the amount of data that can be included in the history. Physical Therapy Reasons for Visit

In the Physical Therapy template set, certain reasons for visit use the dynamic HPI template Reasons for Visit. The dynamic HPI view and the laterality or non-laterality view that appears is based on the particular History of Present Illness (HPI) parameter being documented. Physical Therapy HPI parameters that involve location for a chief complaint related to extremities (for example, pain or numbness related to a hip or knee) appear in the laterality view. Parameters that refer to the chief complaint overall, such as status or place of injury, appear in the non-laterality view. Reasons for visit that do not involve laterality, such as lumbar spine, use the non-laterality view for all HPI parameters. Treatment visits can be based on the laterality or non-laterality view based on the selected reason for visit and the HPI element being documented. Pediatrics Well Child Reasons for Visit

The Well Child visit in Pediatrics enables you to document age-specific complaints for the following reasons for visit: Well Child birth to 1 Year Well Child 1-4 Years Well Child 5-10 Years Well Child 11-21 Years Note: This topic provides information on only those reasons for visit that opens dynamic HPI template associated with Pediatrics, Well Child visit. Treatment visits can be based on the non-laterality view as most patients do not have an issue that pertains to one side of the body or the other. Ambulatory Surgery Reason for Visit

In Ambulatory Surgery Center (ASC), the procedures to be performed on the patient form the reasons for visit. In the Ambulatory Surgery specialty, the procedures documented in the framework templates are used as the reasons for visit that are linked to the dynamic HPI template. The procedure details documented in the Procedure panel appear in the Reason for Visit panel on the ASC - Physician Procedure template. The procedure(s) in the Reason for Visit panel grid opens the dynamic HPI template to add or update additional details on the procedure. Parameters of Ambulatory Surgery The Procedure and Indications parameters of the ambulatory surgery specialty are associated with the findings to document the procedure details. Pediatric Sub-Specialty Reasons for Visit

The Pediatrics sub-specialty Reason for Visit panel includes a list of chief complaints that are configured to launch the following dynamic HPI templates: Autism Chest pain Fibromyalgia Joint pain Juvenile Idiopathic Arthritis Med check Palpitations Scleroderma Note: Not all of the reasons for visit associated to Pediatrics use a dynamic HPI template. Many chief complaints still open a reason-specific HPI template that is not dynamic.
