Erase Concatenated Data in Dynamic HPI

You can erase the concatenated data from the HPI note using the Remove HPI Findings template. You can remove an entire section or a specific finding. On the dynamic HPI, click the Remove button. The Remove HPI Findings template appears. Remove HPI Findings The Click the Side filter field to filter the grid to view lateral information, if applicable. To remove an entire section from the concatenation, highlight the row from the Select the section to remove grid, and click Remove. Select the section to remove from Remove HPI Findings To remove a specific finding, highlight the finding in the Select the finding to remove grid and click Remove. Remove HPI Findings Click the Close button to return to the HPI template.Data is saved upon concatenation to the History of Present Illness column in the Reason for Visit grid on both the HPI and SOAP templates. Document with a Non-Laterality View

Open the Intake template and enter a Specialty and Visit Type, for example, Gastroenterology and Office Visit. Click the SOAP tab, expand the Reason for Visit panel, and select GERD. The dynamic HPI appears with the selected item in the Reason field. To access the other documented reasons in the Reason for Visit panel, click the right or left arrows in the top ribbon. Click Intro in the left pane and select a finding to populate the concatenation field. An asterisk indicates that the selected item has been added to the concatenation. The selected data also fills in the History of Present Illness column on the shared grid that appears on both the HPI and SOAP templates. Click another section in the left pane to continue building the dynamic HPI based on the available findings. The left pane can include free text an entry as an option. When you select free text an entry, a template appears to enable you add additional information to the concatenation. The bottom field fills with the conc

Dynamic HPI Views

There are two views of the dynamic template that are based on data (not specialty content): Laterality view - enables documentation of body parts on one side of the body or the other in two separate grids Non-laterality view - enables documentation of body parts in a single grid Both views use the same template shell. The information that appears in the Reason for Visit grid and in the document depends on the data that is selected. The left pane in each dynamic HPI template lists the sections available for the reason for visit. When a section is selected, the findings associated with that section appear in the right grid. The bottom grid concatenates values based on the sections and associated findings documented for the reason for visit as they are selected. Open the Intake template and enter a Specialty and Visit Type.Enter Physical Therapy for the Specialty and Physical therapy evaluation for the Visit Type. In the Reason for Visit panel on the Objective tab, do one of the following to open the corresponding HPI template: If no Reason for Visit was entered during the intake process, select a reason for visit value (for example, elbow) . If a Reason for Visit appears in the grid, click the desired row. In other specialties, the Reason for Visit panel is also available on the SOAP tab. At the top of the dynamic HPI template, a ribbon field appears with any other documented reasons. To change the Reason on the dynamic HPI template, do one of the following: Click the blue arrow next to the Reason field to select a value from the list. In the top ribbon, click the arrow to toggle and select a previously documented reason and move that value to the Reason field. Click intro in the left pane. Each reason includes an intro. Dynamic HPI Templates

In the NextGen® Adaptive Content Engine templates, the following types of History of Present Illness (HPI) templates are used to document a patient's reason for visiting the office: Static HPI - displays hard-coded content that is not easily customizable; a single template is available for each complaint with options for selecting pre-determined details: the final concatenation is available when you close the template Dynamic HPI - enables you to add, edit, update, and delete dynamic content for the HPI and concatenates the content on the template as items are selected or edited. NextGen® Adaptive Content Engine is packaged with pre-configured dynamic HPI templates for the Gastroenterology, Physical Therapy, Orthopedics, Urology, and Pediatrics Well Child. Categories in History Summary

You can document various history details for a patient in different categories in the History Summary panel. The left pane of History Summary panel on the Intake template includes a list of categories to select and view the documented data. The following table provides a description of each category. Category Description Past Medical/Surgical Enables you to document the patient's past medical and surgical history. You can document the patient history using Add. If no medical or surgical history exists, you can select the No relevant past medical/surgical history checkbox. You cannot select the checkbox if medical surgical history present in the grid. Interim Enables you to document the interim history. Social Enables you to document the Social history. You can select an option from the left pane using Add/Edit.
