Dynamic HPI Templates

In the NextGen® Adaptive Content Engine templates, the following types of History of Present Illness (HPI) templates are used to document a patient's reason for visiting the office: Static HPI - displays hard-coded content that is not easily customizable; a single template is available for each complaint with options for selecting pre-determined details: the final concatenation is available when you close the template Dynamic HPI - enables you to add, edit, update, and delete dynamic content for the HPI and concatenates the content on the template as items are selected or edited. NextGen® Adaptive Content Engine is packaged with pre-configured dynamic HPI templates for the Gastroenterology, Physical Therapy, Orthopedics, Urology, and Pediatrics Well Child. Templates Used to Customize a Dynamic HPI

You can manage History of Present Illness (HPI) sections and findings in the dynamic format on five practice templates and one system template. Preset Data Included on a Dynamic HPI

The dynamic HPI template enables you to add the following patient data into the dynamic HPI framework: View previous chart summaries without leaving the dynamic HPI template Data from the Orders module and Diagnostics template Family history Historical HPI Data from the Orders module and office labs Previous and current medications Past medical and surgical history Questionnaire data The items listed above can be added as Sections on the Dynamic HPI Config template. Laterality View of Dynamic HPI

The documentation of limbs on one side of the body or the other is accomplished in the laterality view on the dynamic History of Present Illness (HPI) template. This view uses two separate grids. Non-Laterality View of Dynamic HPI

The non-laterality view contains a single grid and a field that fills with the concatenated text. This text appears in the Reason for Visit grid and on the generated documents. When you click Intro, the options available for the introductory text varies based on the chosen Reason. In addition, the sections available in the left pane are different for each Reason. These sections of the HPI enable you to dynamically build the historical note. An asterisk at the beginning of the section indicates that it is added to the concatenation. The selected data also populates in the History of Present Illness column on the shared grid that appears in both the HPI and SOAP templates. The concatenation functionality is set up so that you can skip sections in the left pane that are not needed. There are no rules pertaining to the order of selection or the amount of data that can be included in the history. The Chart Summary link in the History of Present Illness panel opens the Chart Summary template

Erase Concatenated Data in Dynamic HPI

You can erase the concatenated data from the HPI note using the Remove HPI Findings template. You can remove an entire section or a specific finding. On the dynamic HPI, click the Remove button. The Remove HPI Findings template appears. Remove HPI Findings The Click the Side filter field to filter the grid to view lateral information, if applicable. To remove an entire section from the concatenation, highlight the row from the Select the section to remove grid, and click Remove. Select the section to remove from Remove HPI Findings To remove a specific finding, highlight the finding in the Select the finding to remove grid and click Remove. Remove HPI Findings Click the Close button to return to the HPI template.Data is saved upon concatenation to the History of Present Illness column in the Reason for Visit grid on both the HPI and SOAP templates.
