Document the Pre-Procedure Details for Hysteroscopy, Ablation, or Sterilization

You can document the pre-procedure details of Hysteroscopy, Ablation, or Sterilization in the Pre-Procedure section on the Hysteroscopy - Procedure template.

1. Open the Hysteroscopy - Procedure template.
2. In the Pre-Procedure section, enter the following details:
   

   • Current contraception
   • LMP

     If the LMP is entered on the Vital Signs template, LMP appears automatically.

   • Assistant
   • Chaperone
   • Sonographer
   • Transportation
3. Optional: Select the Negative pregnancy test checkbox.
4. In Pre-op medication, select the applicable medications in Med #1, Med #2, Med #3.
5. To enter the time of medication, do one of the following:
   • To enter the current time, select .
   • To enter the time using the Time Entry template, select the .
   • Enter the time in Time given.
6. In Pre-Procedure care, select the pre-procedure care details.
7. To enter consent details, select Consent.
8. To enter or view additional screening or sleep apnea assessment documents, select the Configurable Questionnaire links to open the Additional Visit Information template and document the details.

9. To enter or edit the ASA physical status classification, select a classification in ASA physical status classification.
