Document Pregnancy Outcome

You can document the pregnancy outcome in the Pregnancy Outcomes template in the Postpartum tab. Providers use and complete the postpartum template. Select the Postpartum tab. Select Pregnancy Outcome link.The Pregnancy Outcomes template opens. Pregnancy Outcomes template Note: The nurse or medical assistant uses the link and the provider uses the panel to document instead. On the Pregnancy Outcome panel, you can highlight a line in the pregnancy grid and print out a summary of this pregnancy in a newly-created document. This is done when all of the information is entered for the particular pregnancy by the provider. Document the pregnancy entry type, pregnancy number, and termination type/type of delivery at a minimum. However, you can also select to enter the date, sex of the baby, whether full term or preterm, and complications. Documenting Obstetrics Postpartum Visits

You can enter postpartum details in the OB Postpartum template. The OB Postpartum template is used during a postpartum visit. To document a postpartum visit, you must select Obstetrics as the specialty and OB - Postpartum or Document Pregnancy Outcome as the visit type. Note: The OB Postpartum visit type can only be used after every pregnancy. If a patient is seen for a wound check after delivery, you must select the Post-Operative Visit. If a patient has just delivered and the practice want to document the details of the delivery immediately, you can select the Document Pregnancy Outcome visit type. The OB Postpartum template includes the addition of the Postpartum Details and Pregnancy Outcome panels. Compiling this data into a single template improves the workflow. Document Vital Signs in the OB Postpartum Template

You can document vital signs in the OB Postpartum template. Select the Postpartum tab. Select Add in the Vital Signs panel.The Adult Vital Signs template opens. Enter the Vital Signs information. Document Reason for Visit in the OB Postpartum Template

You can enter information in the Reason for Visit panel on the OB Postpartum template. Select the Postpartum tab. Open the Reason for Visit panel. Enter any additional text in the Introduction field.The Introduction field automatically displays a statement describing the patient's age, gender, and reason for visit. The same statement appears on the Reason for Visit panel of the SOAP template. To view the nurse or medical assistant's comments, select the Intake Comments link to open the Intake Comments template. Do one of the following: Select the postpartum check reason for visit from the left pane. Select the Additional / Manage link, and then select the postpartum check option in the Reason for Visit template. Open the Postpartum Details – HPI template and enter the required information. You can document pregnancy outcome in the OB Postpartum template. Select the Postpartum tab. In the Pregnancy Outcome panel, select Add/Update.The Pregnancy Outcomes template opens. Enter the applicable information in the Pregnancy Outcomes template.The entered information displays in the Pregnancy Outcome grid. Verify the parity values are correct. Document Physical Exam in the OB Postpartum Template

You can document physical exam in the OB Postpartum template. Select the Postpartum tab. Select the Physical Exam panel. Do one of the following: Select the Postpartum Exam option from the left pane. Select the Additional link, and then select the Postpartum Exam option from the picklist. The Postpartum Exam template opens. Select the One Page Exam link.The GYN-PE template opens. Enter the applicable information in the General Exam section in the Postpartum Exam template. If you select the All others normal/negative checkbox, the Normal checkbox is selected for all the fields. Enter the applicable information in the Breast Exam section. Select the Breast Exam link. In Breast- Physical Exam, if you select the All others normal checkbox, the Normal checkbox is selected for all the fields. Select the Genitourinary link.
