Document Interpretations of Holter Monitor Tests

You can document the interpretations of the Holter monitor test in the Doctor Interpretation section. Open NextGen® Enterprise EHR. On the Intake tab, select Cardiology as Specialty and Testing Only as Visit Type. Click the Holter link. You can also access the Holter Monitor template from the navigation bar. The Holter Monitor template opens. Click Baseline Rhythm and select an underlying rhythm. Enter Other Rhythm Comments, if any. Click Atrioventricular Block and select an option. Enter the details of Supraventricular Ectopy and Ventricular Ectopy. Click ST Segment Changes and select an ST change. Enter the Symptoms and Final Comments. Click Save. Document Holter Procedures

You can record the Holter Monitor test details. The Intake staff documents the procedure information which includes pre-procedure care, test/patient data, vital signs, stress data, recovery data, and wall motion. Open NextGen® Enterprise EHR. Click the Intake tab. Select Cardiology as Specialty and Testing Only as Visit Type. Click the Holter link. Note: You can also access Holter Monitor template from the left Navigation bar. The Holter Monitor template opens. Holter Monitor-Procedure Information Click the Procedure Information panel. Click the Indication field and select an indication. Click the Hook Up Date field and select the date when the holter monitor has been hooked. Click the Disconnect Date field and select the date when the holter monitor has been disconnected. Click the Send Copy To field and select a physician to whom you want to send a copy of the test report. Click Ordering Dr and select the ordering physician. Click Diary and select an option. Document Summary of Echocardiogram Tests

You can document a summary of the Echocardiogram test in the Summary panel. Open NextGen® Enterprise EHR. Click the Intake tab. Select Cardiology as Specialty and Testing Only as Visit Type. Click the Echo link to access the Echocardiogram template. In the Summary panel, click All Normal if all the results are normal. The Summary fields display the default statements for normal results. Enter a brief summary in the Summary field. Select an appropriate option from the list. Click the S icon to frame a sentence using the Sentence Builder template. Parent topic: Echo

Document Findings of MPI Tests

You can document the findings of the MPI test in the Findings section. Open NextGen® Enterprise EHR. Click the Intake tab. Select Cardiology as Specialty and Testing Only as Visit Type. Click the MPI link. Note: You can also open the MPI-Card template from the left Navigation bar. The MPI-Card template opens. MPI-Card-Findings To select all the findings of the test as normal, click All Normal. Select the appropriate option from the Confirm Prior Study. If you click Yes, a Prior Study Comparison template opens. If the results are not normal, select appropriate options as per the findings and enter comments wherever necessary. Click Save. Document Blood Pressure and Pulse

You can measure a patient's blood pressure and pulse. Open NextGen® Enterprise EHR. Open a patient's record, and then open an encounter. On Top Toolbar, select Templates . The Select Template window opens. Open the Intake template. In the Vital Signs panel, select Add. The Pediatric [Age] Vital Signs template opens. Enter the blood pressure reading in the Systolic and Diastolic fields within the 0 – 399 range. Note: If the readings are out of the normal range, a message appears. For more information on blood pressure alert messages and how to configure the alert messages, go to NextGen Healthcare Success Community, and download the Vital Signs Configuration Practice Template for NextGen® Adaptive Content Engine. Select a position and side. Enter the site where the blood pressure reading was taken. Select a method, and then select the appropriate cuff size. Document Information with Template Screening Tools

You can document information in the screening tools that open as templates using the Screening Tools template. Open NextGen® Enterprise EHR. Select a patient encounter, and open a template. For example, Intake. Select Navigation, and then select Screening Tools. The Screening Tools template opens. To view the screening tools, do one of the following: To view the screening tools configured for the selected specialty, select Current Specialty. To view all the screening tools, select All. Note: When you do not select a specialty for an encounter, the All option is selected by default and all the screening tools appear on the Screening Tools template. In the Screening Tools Available panel, select a screening tool with the T indicator. The associated screening tool template opens. Document details provided by the patient.
