Social Confidential History

The Social Confidential history configured in the Practice Configuration and Document Configuration practice templates displays in the documents. The Confidential Social history can be configured to always display in the document in the Practice Configuration template. If the setting in the Practice Configuration template is not set to automatically display the confidential social history, then this information will not display in the document unless the checkbox is selected in the Confidential Social History template. Social History

You can configure options in the Social History section. The Social History section allows you to configure the following: Hide Firearms - If you select the Hide Firearms checkbox, the system hides the firearms options and link on the Social History - Lifestyle template. The Firearms at home options and Firearms link are available on the Social History - Lifestyle template. Adult Social History template Show Confidential History - If you select the Show Confidential History checkbox, the system automatically selects the Include all confidential information in the social history document with patient permission checkbox on the run-time confidential history template. If you select the Show Confidential History checkbox, the system automatically selects the Show Confidential History Information on Master Document checkbox on the Document Configuration template. Confidential Information

You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the following links. The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. The links allows you to document the following confidential information of the patient. Alcohol and drug use On the Confidential - Alcohol/Drug template. Psychiatric history On the Confidential - Psychiatric template. Abuse or domestic violence and incarceration On the Confidential - Abuse/Incarceration template. Sexual practices and STI details On the Confidential - Sexual Practices/STIs template. With the patient's permission, you can generate the documented confidential details in the social history document. You need to select the Include all confidential information in the social history document with patient permission checkbox on the templates to include

Pediatric Confidential Information

You can use the confidential history templates to document a patient's confidential details. You can open the Peds Confidential History template by clicking the following links: The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. You can document the following confidential details of the pediatric patient: Alcohol and drug use, and any related comments from the historian on the Peds Confidential History - Substances template. Psychiatric history, child placement history, and incarceration details from the historian on the Peds Confidential History - Psychiatric template. Sexual practices, Sexually Transmitted Infection (STI) details, and any related comments from the historian or the provider, or from both on the Peds Confidential History - Sexual Hx template. Document Confidential Psychiatric Information for Adult Clients

From the left pane on the Confidential – Alcohol/Drug template, click the Psychiatric link. The Confidential – Psychiatric template opens. Confidential - Psychiatric Select the Noor Yes option for the following: History of suicidal thoughts History of homicidal thoughts Diagnosed with psychiatric problem Enter the Psychiatrist name, Therapist name and corresponding Phone number. Select Yes or No for the following queries: Consent for communication between psychiatrist and PCP Consent for communication between therapist and PCP Family Hx of psychiatric problems Select the Abuse/Incarceration, Sexual Practices/STI's, and Provider Comments links from the left pane to document the required details. If the patient provides permission to include the documented information in the social history document, select Include all confidential information in the social history document with patient permission at the top of the template.
