Calculating the Framingham Coronary Heart Disease Risk

You can calculate the Framingham Coronary Heart Disease Risk using the Screening tools template on the navigation bar. You can document the screening details of risk stratified cholesterol (LDL-C) and calculate the risk for coronary heart disease using the Framingham Risk Calculator. Before using the risk calculator, add the patient's systolic and diastolic blood pressure details for the encounter in the Vital Signs panel. Note: Framingham Risk Calculator is not appropriate for patients less than 20 years of age and those with diabetes or known coronary heart disease. Cardiology Provider Workflow

Provider reviews the patient's summary on the Home Page and proceeds to the SOAP and the Finalize templates. The provider workflow consists of the following: Reviewing the data documented up to this point in the encounter to obtain an overview of the patient's condition. Viewing Risk Factors

You can view details about patients' risk factors to help you to evaluate their health condition. The risk indicators for Tobacco use (TOB), Hypertension (HTN), Diabetes (DM), and Coronary artery disease (CAD) are available for documentation in NextGen® Adaptive Content Engine. Risk factors You can use the Risk Factors Config template to indicate a patient's current health conditions. You cannot consider a patient's health at high risk by looking at these conditions or you can view the risk indicator icons without opening the template. If you want to indicate that the patient is at high risk for developing a certain condition, you can document such details on the Care Guidelines template. Document the Risk Factors

You can document the risk factors for patients. Open NextGen® Enterprise EHR. On a patient encounter, open any of the standard visit templates. (for example, the Intake template. On the template header, select Config. The Risk Factors Config template opens. If the patient has no risk factors, select No risk indicators, and select Save & Close. If the patient has a history of tobacco use, do the following: In the Tobacco section, select Tobacco Usage.The Social History - Tobacco template opens. Note: If the tobacco information is updated on the Screening Summary template or on the Risk Factors Config template, the application automatically fills values on the Social History - Tobacco template. Enter data on the Social History - Tobacco template, and select OK. Select the Tobacco Cessation Discussed link.The Tobacco Cessation Discussed template opens. Screen for Future Fall Risk

You can document the patient's risk for falls. Open NextGen® Enterprise EHR. To open the Social History – Lifestyle template, do one of the following: On the Histories template, on the Social panel, select Lifestyle , and select Add/Edit. In the History Summary panel on Intake, select Lifestyle from Social, and select Add/Edit. The Social History – Lifestyle template opens. In the Home Environment/Safety section, in Falls in the last year?, select the applicable option. If you select Yes, enter the number of falls. In Did the fall(s) result in injury?, select the applicable option. If you select Yes, then enter information about any injuries resulting from falls in the Details field. In Is the patient at risk for falls?, select the applicable option. Select the Fall Risk Plan link to access the Fall Risk section of the Health Promotion Plan template, if needed. Chronic Conditions and Status

Under the 95 and 97 Coding Guidelines, the E&M calculation for chronic conditions occurs when you add a Chronic Condition and Status to the Today's Assessment field. The application assigns a field value of 1 to the shared audit fields for each chronic condition documented with a status. The detailed E&M history audit fields on the Finalize template also track where the chronic conditions were documented. E&M Level Summary The Chronic Conditions field in the E&M Level Summary panel indicates the total chronic conditions count. If the count is 3 or more, the application calculates an extended HPI. The Chronic Conditions From Today's Assessment field in the History Details panel totals the Chronic Conditions with a documented Status that were added to Today's Assessment.
