Chronic Conditions Data
The Chronic Conditions and Chronic Conditions HPI templates are used for documentation of multiple chronic conditions in one place, including diabetes. In addition, you can review health maintenance items, order tests, and review recommended care items and patient goals and/or self-management. Note: The Chronic Conditions and Chronic Conditions HPI templates cannot be used on the same encounter. To use the Chronic Conditions template, be sure that chronic conditions is not selected in the Reason for Visit panel. The Chronic Conditions and the Chronic Conditions HPI templates also provide access to the following areas: Flowsheets Health Promotion Plan Lab results and review Medication review and reconciliation Medication history Access the Chronic Conditions Template Access the Chronic Conditions - HPI Template Parent topic: Diabetes Care

Chronic Conditions
You can view the chronic condition details of a patient from the Reason for Visit panel. The Chronic Conditions template allows you to document the chronic conditions for the patient. In addition, you can view and associate the patient’s active medications with three active diagnoses (Diagnosis 1, Diagnosis 2, or Diagnosis 3) using the Associate Meds link available in the Chronic Conditions template. You can select the Chronic Conditions button in the Reason for Visit section to open the Chronic Conditions template. To remove the chronic condition entry from the grid, you must select the row containing the chronic conditions record, and then you must select Remove. Document Chronic Conditions You can document patient's chronic condition details in the Reason for Visit panel. Associate Medications The Associate Medications template allows you to view and associate an active medication with up to three active diagnoses. Document Lab Glucose Results
On the Chronic Conditions - Hpi template, select the Lab Review link.The Lab Review template displays external and in-house lab results. Enter lab results data. Enter Medication History on the Chronic Conditions - HPI
On the Chronic Conditions - Hpi template, navigate to the Medications section. Select History.The Medication History template opens. This is a convenient place to see medications given in the past for the same condition, and whether or not they were efficacious. Access the Health Promotion Plan
Do one of the following: Select the Health promotion plan link on the Chronic Conditions - Hpi. Select the Health promotion plan link on the Screening Tools template. For more information on Screening Tools template, go to the NextGen Healthcare Success Community, and download the Getting Started User Guide for NextGen Adaptive Content Engine.
