Document Health Promotion Plans

You can document the Health Promotion Plans such as BMI plan, depression plan, and hypertension plan of a patient. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Health Promotion Plan panel. Add a diagnosis. Select Plan and select a plan from the list.Based on the selected plan, the different sets of fields appear. To add the plan details, do the following: For BMI Plan, add the diet, physical activity, and referrals details. For Depression Plan, dd the follow-up and referrals details. For Hypertension Plan, add the diet, physical activity, referrals, and lifestyle details. Note: To turn on the Referrals section and order a referral, you must add a diagnosis. To indicate that the referral is internal, select the Internal referral checkbox. If required, in the Details field, enter any additional information. If you want to add or update patient educat

Health Promotion Plans

The Health Promotion Plan template is used to document screening and a follow-up plans of care for Quality Reporting Care Plans, which are required for successful Quality Reporting. The template includes the following types of Quality Reporting Care Plans: BMI Health Promotion Plan Depression Health Promotion Plan Hypertension Health Promotion Plan The template also includes additional panels for documenting the screening and follow-up plan of care for pain management, fall risk, and functional status for successful Quality Reporting. NextGen® Adaptive Content Engine allows you to create customizable plans. Activate Diabetes Care Guidelines

Care guidelines can be set up for diabetes. The guidelines include tests, as well as recommended care items, and are a form of clinical decision support. Practices can customize care guidelines on the Activate Care Guidelines template. For more information on configuring care guidelines, go to NextGen Healthcare Success Community, and download the Care Guidelines User Guide for NextGen® Adaptive Content Engine. On the Chronic Conditions – Hpi template, navigate to the Care Guidelines section and select Update.The Activate Care Guidelines template opens. Select Add/Update in the Care Guidelines to add new guidelines or edit existing ones. Selecting the Diabetes link in the Care Guidelines template displays the configured diabetic guidelines. Select OK to return to the Chronic Conditions - Hpi template.Once the clinical or care guidelines are configured, they can be viewed and updated on the Chronic Conditions – Hpi template. Set Up HPIs to Appear in the Reason for Visit

You can set up the History of Present Illness (HPI) to appear in the reason for visit. A practice can configure the Reason for Visit panel using the Framework Content system template. For more information on the reason for visit configuration, go to NextGen Healthcare Success Community, and download the latest Framework Content System Template for NextGen® Adaptive Content Engine. Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. In the Show section, select All. To open the Framework Content template, double-click Framework Content. Select Specialty and select a specialty from the list. Expand the Reason for Visit panel. In Filter, select the all option. Select the reason for visit linked to the appropriate History of Present Illness (HPI), which you want to add to the all list in the Reason for Visit list.
