Document Patient Reported Glucose Results

On the Chronic Conditions - Hpi template, navigate to the Labs section. To document and graph the patient’s reported glucose results, select the Home Glucose Readings link.The Home Glucose Map template opens. To enter values, do one of the following actions: Select Download Meter Data. Select the Date field to enter the client reported value and continue to enter data points in the resulting windows. To create a visual representation of the data, select the Graph Daily Readings and Graph Min., Max. & Average. Select Save & Close to return to the Chronic Conditions - Hpi template. Chronic Conditions Data

The Chronic Conditions and Chronic Conditions HPI templates are used for documentation of multiple chronic conditions in one place, including diabetes. In addition, you can review health maintenance items, order tests, and review recommended care items and patient goals and/or self-management. Note: The Chronic Conditions and Chronic Conditions HPI templates cannot be used on the same encounter. To use the Chronic Conditions template, be sure that chronic conditions is not selected in the Reason for Visit panel. Re-applying a Guideline

You can re-apply a care guideline for a patient in the Activate Care Guidelines template. You are re-applying a guideline when you activate an already active guideline. You can re-apply a guideline to follow new items within the guideline. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines template opens. Select Add/Update in the Care Guidelines panel. The Activate Care Guidelines template opens. In the Active Guidelines panel, select the Condition field. A list of all pre-configured care guidelines opens. Select the condition in the list for the guideline to re-apply and select OK. A message appears asking if you would like to re-apply the care guideline. Select Yes to re-apply the guideline, or No to cancel. Parent topic: Activating a Care Guideline

Setting Up Care Guidelines for Use

You can set up the care guidelines for a patient and document the details on the Care Guidelines template. In NextGen® Adaptive Content Engine, the Care Guidelines template is used to handle Health Maintenance and Disease Management functions, including guideline testing and screening, and recommended care. You can open the Care Guidelines template using the Care Guidelines link, in the top navigation tabs of all the main tabbed templates for every specialty. CAUTION: Before using the Care Guidelines template in a clinical setting with patients, content on the template should be carefully reviewed by the individuals who are responsible for maintaining the system. You can update as necessary to meet the needs of the practice. Care Guidelines link is always available on the Care Guidelines template. Activate the Care Guideline You Want to Follow for the Patient

You can add care guideline details for a patient record using the Care Guidelines panel. Open NextGen® Enterprise EHR. Open the Care Guidelines template. In the Care Guidelines panel, select Add/Update.The Activate Care Guidelines template opens. In the Active Guidelines panel, select the Condition field.A list of all pre-configured Care Guidelines appears. Select the pre-configured condition in the list and select OK. If applicable, select the Diagnosis field to select a diagnosis in the diagnosis search window.The Diagnosis field is used to associate a diagnosis with an ICD code (used for billing) to the guideline. The selected diagnosis appears in the Diagnosis field and the associated code appears in the Code field. Select Add.The guideline activates, and items that are associated with it appear in the grid on the Active Guidelines panel.
