Demographic Information for UDS

The following demographics information must be collected on the Demographics tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM: Birth Date/Age Sex Race Preferred Language Ethnicity Zip Code The following UDS information must be collected on the UDS tab of the Add/Modify Patient Information window for all patients in NextGen® Enterprise PM: Homeless Status Migrant Worker Status Language Barrier Public Housing Primary Care School Based Health Center Veteran Status Primary Medical Coverage Note: The Race and Ethnicity fields can display on either the Demographics tab or the UDS tab of the Add/Modify Patient Information window. In Enterprise Preferences > Client Defined, select Display General to display race/ethnicity on the Demographics tab, or select Display UDS to display race/ethnicity on the UDS tab. Family Information for UDS

Family size and annual income are defined for a head of household. The following information must be captured on the UDS tab of the Add/Modify Head of Household Information window in NextGen® Enterprise PM for Table 4 in the UDS report: Family Size Annual Income Parent topic: Information Needed for the UDS Report

Family Information

In the Family Information section of the UDS tab, you can enter family size and annual income for the patient's head of household. This information is typically used by Community Health Centers in conjunction with sliding fee schedules. When a patient qualifies for a sliding fee schedule, family size and income are used to determine the percentage of charges that are automatically adjusted for the patient. Family information is also reported on the UDS Report. Note: A head of household must be selected for a patient to enter family information. Family Information is stored at the practice level; therefore, each practice can enter and maintain different information for a person or patient. Information Needed for the UDS Report

Although some data required for the UDS Report is more readily (or only) available from a general ledger or other external system, much of the data can be reported directly from NextGen® Enterprise PM. Several demographic fields required by Health Centers for the UDS report are available on the UDS tab of the Add/Modify Patient Information window. Additional fields are available for use by IHS practices. To display the UDS tab, enable the Display person UDS fields check box in Enterprise Preferences > UDS.
