If you are the administrator, you can add or remove placeholders in the form template for cost estimation. You can also specify if the form requires electronic signature.
 
 
  - In File Maintenance, select . 
   
    The Forms Enhanced window opens.
   
 
  - Select or create a form for cost estimation.
   
    The form template opens.
   
 
  - Expand Pre-Service/Estimate Information to view the Pre-Service fields. 
 
  - On the draft form template, place your cursor where you want to add a placeholder, and double-click a Pre-Service/Estimate Information field in the right pane. 
   
    The placeholder appears in the form template. 
    
Note: The data for the form is sourced from the Estimate Patient Cost window.
    
 
    
  - For an electronically signed cost estimation form, select the Required Esignature option in the Esignature Options section.
 
  - Save your changes.
