Budget Statement Headers and Footers

Budget statements have a standard Remittance Header and Default Footer as shown below.

The standard remittance header includes the following information:

  • Make Checks Payable To section with practice name and address
  • Addressee section with guarantor name and address
  • If Paying by Credit Card section
  • Remit To section with practice name and address

The standard default footer includes the following information:

  • Account Number, Aging Categories, Total Account Balance
  • Message: Default footer message from Practice Preferences > Statements
  • Please Pay This Amount
  • Payment Due Upon Receipt banner
  • STATEMENT
  • Page n of n

By default, the remittance header and default footer print on every page for budget statements. This may increase the number of pages printed when generating budget statements, particularly for practices that have many large guarantor/patient accounts with multi-page budget statements.

Whether the standard header and footer print on every page for multi-page budget statements can be controlled by settings in Practice Preferences > Forms.

Display Remittance header on first page only:

If not selected (default), the remittance header prints on all pages for multi-page budget statements.

If selected, the remittance header prints on the first page only for multi-page budget statements. The header on subsequent pages includes the following information:

  • Practice Name
  • Account Name
  • Account Number

Display default footer on last page only:

If not selected (default), the default footer prints on all pages for multi-page budget statements.

If selected, the default footer prints on the last page only for multi-page budget statements. The footer on prior pages includes the following information:

  • STATEMENT
  • Page n of n
