Create Cost Estimation Forms in File Maintenance

If you are the administrator, you can add or remove placeholders in the form template for cost estimation. You can also specify if the form requires electronic signature. In File Maintenance, select Master Files > Systems > Forms Enhanced. The Forms Enhanced window opens. Select or create a form for cost estimation. The form template opens. Expand Pre-Service/Estimate Information to view the Pre-Service fields. On the draft form template, place your cursor where you want to add a placeholder, and double-click a Pre-Service/Estimate Information field in the right pane. The placeholder appears in the form template. Note: The data for the form is sourced from the Estimate Patient Cost window. For an electronically signed cost estimation form, select the Required Esignature option in the Esignature Options section. Save your changes. Print Cost Estimation Forms

Before you begin Ensure that your administrator has customized the cost estimation form template in File Maintenance. There are many ways in which you can initiate the printing of the cost estimation form. Do one of the following to initiate the printing of a cost estimation form. To print Action Cost estimation form from the current cost estimation (without electronic signature) On the Estimate Patient Cost window, specify and submit the data for cost estimation. Select Print. Use the search criteria to find the desired encounter. Right-click the encounter, and select Print > Forms. After capturing the electronic signature on the cost estimation form, close the form toolbar. A message appears to confirm that you want to archive the signed document. Note: You can archive forms that contain at least one electronic signature. The message does not appear if there are no electronic signatures on the form. Select Yes. Printing Estimated Patient Cost

You can obtain a printed output of the cost estimate. The form can be created according to the needs of the patient or person, insurance provider, rendering provider, and so on. Your system administrator can create the form template using the Pre-Services/Estimate Information fields in File Maintenance > Form Enhanced. The form can be printed with or without electronic signature. For electronically signed forms, your administrator must enable the electronic signature feature in File Maintenance. After printing a form, it can be archived only if it is electronically signed. You can view and reprint the previous cost estimation forms from the estimation results in the Patient Chart window. Note: If your administrator has modified the form template, you will get the output in the modified format when you reprint it from the estimation results in the patient's chart. Archive Electronically Signed Forms

You can archive electronically signed enhanced forms. After capturing electronic signatures on the form, select the Close icon on the form toolbar. A message stating Do you want to archive this signed document? appears. Note: You can archive forms that contain at least one electronic signature. The message does not appear if there are no electronic signatures in the form. To archive the electronically signed form, select Yes.
