Add Diagnosis Codes to a Case
You can select diagnostic and procedural ICD codes to apply to your cases.
Enter all required and optional General Case Information.
Select the
Select Diagnosis/Procedure
button.
The
Diagnosis Selection
window opens.
Select the Diagnostic or Procedural option button.
Search for the desired ICD Code.
If necessary select
Search for keyphrase anywhere in diagnosis
or Search Patient
NextGen® Enterprise EHR
Problem List options.
Select the desired ICD Code.
Select
Move Right
.
Repeat Steps 3 through 7 to add additional ICD codes, if necessary.

Select Diagnosis and Admitting Diagnosis for a Procedure
You can either enter or search and select a diagnosis for a patient's procedure. You must select at least one diagnosis for the patient. To enter up to 12 diagnoses for a procedure, you must select Allow twelve diagnoses under the EHR tab in thePractice Preferences window in File Maintenance. Open NextGen® Enterprise EHR. Open Procedures window in Procedures modules. In Diagnosis field, enter a diagnosis. If a diagnosis code does not exist in the patient's chart, a message appears confirming that add the diagnosis to the patient's chart. To add the diagnosis, select Yes. To continue adding the procedure without adding the diagnosis to the patient's chart, select No. To select a Diagnosis, select the ellipsis button. The search window opens. Search Select the preference icon and select the columns to display. Search results The selected columns appear in the search window. Options for Adding Diagnosis Codes
In the Diagnosis (Dx) section of Today's Visit, you can select and add multiple diagnosis codes at a time from various options. You can rearrange the order of the diagnosis codes, and update and delete the added diagnosis codes. Note: You can use 25 complimentary diagnosis workflows per month. For unlimited use of diagnosis workflow, you must upgrade to the NextGen® Mobile Plus or NextGen® Mobile Pro package. You can select Contact Me on the notification to request the NextGen Healthcare Sales team to contact you. Diagnosis Options Tabs Description Previous Displays the diagnosis codes previously added to that patient's chart. Popular Displays the diagnosis codes commonly used by the provider. Codes are added to the Popular section after being added for two or more times. Suggested Displays relevant diagnosis codes based on the NextGen® Ambient Assist generated note. Diagnosis Categories Search
From the Master Files - System list, select Diagnosis Categories. The Diagnosis Categories List window opens. To add a new diagnosis category, select Open Menu , then select New. To search for an existing diagnosis category to modify, follow the steps below: In the List Search field, enter what you want to search for. The matching results are listed. Note: You can enter an asterisk ( * ) to see all the records. Double-click the diagnosis category you want to view. The Add/Modify Diagnosis Category Information window opens. Enter a name for the diagnosis category in the Category field. Enter a four-digit abbreviation for the diagnosis category in the Short Description field. To add an ICD code and description select Open Folder . The ICD Codes window opens. To add a code to the category, select the code in the Available list, then select the Include . The code moves from the Available list to the Included list. If necessary, select either 09 or 10 in the Show field to display only ICD9

Search for Diagnoses Codes
Open NextGen® Mobile. Open a patient chart. Select the Diagnostic icon. To add a diagnosis to the encounter, select Add Diagnosis. The Add Diagnosis view opens. Add diagnosis for a patient encounter Enter your search criteria in the Search field. You can enter incomplete phrases. The list of codes matching your query appears. Some codes are multi-tiered, indicated by arrow icon. To view the encounter type or any other category associated with the code, select Arrow icon. A list of the available category types appears. Select the desired category type. The associated codes appear. To add a diagnosis code from the search results list, select the desired diagnosis code or select a diagnosis, and then select Add to add the code to the patient encounter. Add Problems in the Problem List
You can add a problem to the problem list of a patient. Open NextGen® Enterprise EHR. Open the Histories template. In the Problem List panel, select Add/Update. Enter a problem in the diagnosis search window, and select Search. Select the problem from the list of the search result, and select Select. Select the applicable ICD code, and select Select. The application automatically fills the associated values in the Fully Specified SNOMED Description, Problem Description, ICD Description, and ICD Code fields in the Problem List panel. Note: For all new problems, the Mark as Chronic check box is selected by default. In Clinical Status, select the status of the problem. In Onset Date, enter the date of onset. In Resolved Date, enter the date when the selected problem is resolved. If the problem is not chronic, to mark the problem as a secondary condition, select Secondary Condition. Select National Drug Code ID for a Procedure
You can select a National drug Code ID (NDC ID) for a procedure. Open the Procedures window in NextGen® Enterprise EHR. To select NDC ID, select the ellipsis button. The Select National Drug Code ID window opens. Select National Drug Code ID Enter one of the following: Description, NDC ID, CPT4. Select Search. In the results, select the required NDC ID. Do one of the following: To exit with the selected NDC ID, select OK. To empty all search fields and perform a new search, select Clear. To exit without selecting an NDC ID, select Cancel.
