Add or Modify Diagnosis Severities

The Diagnosis Severities master file enables you to set up levels that indicate the severity of a patient's diagnosed condition. Diagnosis Severity is based on user-defined criteria. This master file should contain a comprehensive list of severities. From the Master Files - System list, select Diagnosis Severity. The Diagnosis Severities List window opens. Do one of the following: To add a new item, select open menu , and then select New. To modify an existing item, select the item in the list, select open menu , and then select Open. You can also double-click to open the item. The Add/Modify Diagnosis Severity Information window opens. Enter a full description of the diagnosis severity in the Description field. Note: It is strongly recommended that you set up the first record for severity with a description like None or something similar. Having this "blank" record as the first record in the table enables users to not select a severity if they choose to do so. Enter any additional inf

Diagnosis Categories Search

From the Master Files - System list, select Diagnosis Categories. The Diagnosis Categories List window opens. To add a new diagnosis category, select Open Menu , then select New. To search for an existing diagnosis category to modify, follow the steps below: In the List Search field, enter what you want to search for. The matching results are listed. Note: You can enter an asterisk ( * ) to see all the records. Double-click the diagnosis category you want to view. The Add/Modify Diagnosis Category Information window opens. Enter a name for the diagnosis category in the Category field. Enter a four-digit abbreviation for the diagnosis category in the Short Description field. To add an ICD code and description select Open Folder . The ICD Codes window opens. To add a code to the category, select the code in the Available list, then select the Include . The code moves from the Available list to the Included list. If necessary, select either 09 or 10 in the Show field to display only ICD9

Select Diagnosis Data

Select Diagnosis from the Report Filter Settings List. Report Filter Select to display the Diagnosis Search window. Diagnosis Search Select the pertinent Library. To further narrow your search, select the Category. Select whether you wish to search by ICD or Dx Code. Enter a Diagnosis search criterion. Note: Make sure to use the proper ICD Code for your search. If your Practice is using the decimal ICD Code, make sure to enter the ".". Select Search. Select the desired Diagnoses. Select OK. The selected diagnoses display in the Diagnosis Search pane. Select whether to Include or Exclude the displayed diagnoses. Select whether to display patients who have had problems within specific date criteria, for example: Patients who have never had the problem before or after a specific date or between a specific date range. Patients who have ever had a problem before or after a specific date. Patients who have not had this problem since a specific date. To select a date range, check Who Have Eve

Allergies - User Defined

The Allergies master file contains a list of user-defined allergies. From the Master Files - System list, select Allergies. The Allergies - User Defined List window opens. Do one of the following: To add a new item, select open menu , and then select New. To modify an existing item, select the item in the list, select open menu , and then select Open. You can also double-click to open the item. The Add/Modify Allergy Information window opens. Enter the abbreviation for an allergy in the Allergy Key field. This field allows for a maximum of 8 characters. Enter the full description of the allergy in the Allergy Description field. Enter any additional information for the allergy in the Note field. If you want to hide the item in the list, select the Hide check box. Select Next to add another item or select OK to return to the Allergies - User Defined List window. Modifiers

The Modifiers master file provides a complete list of modifiers in the database. This table may be populated through your billing system. From the Master Files - System list, select Modifier. The Modifiers List window opens. From the Modifiers List window, do one of the following: To add a new item, select Open Menu , and then select New. To modify an item, select the item in the list, select Open Menu , and then select Open. To modify an item, enter an asterisk in the List Search field to display the existing modifiers. Then, double-click the modifier you want to change. The Add/Modify Modifier Information window opens. In the Modifier Key field, enter an industry standard abbreviation (two-character maximum) for the modifier. In the Modifier Description field, enter the full description for the modifier. In the Note field, enter any additional information. If you want to hide the item in the list, select the Hide check box. Select Next to add another item or select OK to return to th

Modify the Billing ICD List Data Entry Fields

You can modify selected entries in the billing ICD list using the billing ICD list data entry fields. You can modify the billing ICD list data entry fields for an ICD selected in the billing ICD list. Open Billing ICD List. In the Billing ICD List data entry area, verify the information entered in the Encounter Date, Code, and Description fields. To select the onset date, select the Onset Date field and use the calendar. Select the Diagnosed Date field and use the calendar to select the diagnosed date. Note: The Diagnosed Date cannot occur before or after the problem's Encounter Date. If necessary, create a encounter to enter a diagnosed date. If you attempt to assign a Resolved Date that is before or after the Onset Date, the application displays an application message.
