Use a Condition Group to Enter Family History

You can select a family health history from a group of health conditions. Open NextGen® Enterprise EHR. Open the Histories template. In the Family panel, and select Add.The Family Health History template opens. Select a grouped condition with a plus (+) sign next to the condition. The Details icon appears next to the condition. Select and select a secondary diagnosis from the resulting list. The Family History Expanded Conditions template opens. On the Family History Expanded Conditions template, do the following: In Onset age, enter the family member's age at the time of diagnosis. If the family member died from the selected condition, select the Cause of death check box. To document any additional information, select the Comments link and enter the details. To return to the Family Health History template, select Save to Grid & Close. You can use Edit or Remove to manage the family history of the patient. Document a New Family History

You can add the family history of a patient from the Family category in the Family panel on the Histories template. Open NextGen® Enterprise EHR. Open the Histories template and then expand the Family panel. In the Family panel, select Add. The Family Health History template opens. Note: NextGen® Enterprise EHR fills Specialty field with the current specialty. The available conditions for the specialty appear alongside a check box on the Family Health History template. To indicate that the patient does not have any family history, select the No family history of check box. In the Relationship field, select a description from the Family relation list. In the Family member name field, enter the name of the selected family member, and do one of the following: If the family member does not have any condition that can be reported, select the Alive and well option. Add a Single Condition to the Family History

You can document the single condition for a patient's family history. Open NextGen® Enterprise EHR. Open the Histories template. In the Family panel, select Add. The Family Health History template opens. Select the check box next to an individual condition. In the Onset age field, enter the family member's age at the time of diagnosis. If the family member died from the condition, select the Cause of death check box. Note: When you select Cause of death, the Age at death field appears to enter the age of a dead person and NextGen® Enterprise EHR selects the deceased field automatically. Select the Comments link. The Family History Comments template opens. Family History Comments Enter any additional information about the family member and select Save & Close. Select Save to Grid. Edit Family History Details

You can update family history details in the Family History Details template. Open NextGen® Enterprise EHR. Open the Histories template. In the Family panel, select the row of a family member, and select Edit. The Family History Details template opens. Family History Details Update Name and Onset age. To update the alive or deceased designation for a family member, do the following: Select Alive and well or Deceased. Note: The Alive and well and Deceased options are only enabled if you select Alive and well at the time of documenting a condition. If you select Deceased then update the Age at death and if the person died with the same selected condition then select Cause of death check box. Note: When you edit the Alive and well or Deceased option for a family member and there are multiple family history records for that family member, you must manually delete the other records and re-enter the conditions with the updated designation for Alive and well or Deceased. Add Family History

You can add the family history of a patient from the Family category in the History Summary panel or in the Family panel on the Histories template. To indicate that no relevant history is available, select No relevant family history. If there is no known family history for an adopted patient, select Adopted - no family history known, and skip documenting the Family category details. Open NextGen® Enterprise EHR.
