Use a Condition Group to Enter Family History

You can select a family health history from a group of health conditions.

  1. Open NextGen® Enterprise EHR.
  2. Do one of the following to open the Family Health History template and document the family history:
    • On the Histories template, expand the Family panel, and select Add.
    • In the History Summary panel on the Intake template, select Family, and select Add.
  3. Select a grouped condition with a plus (+) sign next to the condition.
    The Details icon appears next to the condition.
  4. Select and select a secondary diagnosis from the resulting list.
    The Family History Expanded Conditions template opens.
  5. On the Family History Expanded Conditions template, do the following:
    1. In Onset age, enter the family member's age at the time of diagnosis.
    2. If the family member died from the selected condition, select the Cause of death check box.
    3. To document any additional information, select the Comments link.
    4. Enter the details on the Family History Comments template, save, and close the template.
    5. To return to the Family Health History template, select Save to Grid & Close.
    You can use Edit or Remove to manage the family history of the patient.
  6. Save and close the Family Health History template.
