Add a Treating Diagnosis from the Common Assessments Template

You can add additional treating diagnoses using IMO or Common assessment templates. Note: If your practice is licensed for Problem Intelligent Medical Objects (IMO) functionality, select the Problem IT button under the Treating Dx grid to open IMO to add assessments. Select Add. The Inf Common Assessment window opens. If the Common Assessments field is blank or does not show the Diagnosis Codes page you want, do the following: Select the Common Assessments field. The Inf Common Assess Dbpl window opens. Select the Diagnosis Codes page you want, and then select OK. The selected Diagnosis Codes page opens. Select one or more diagnoses. If you select an item with a plus sign, a picklist opens for you to select a more specific diagnosis. The Treating grid shows all selected diagnoses. Select Save & Close. The diagnosis descriptions and codes that you have selected enter the Treating Dx grid on the Single Visit template. Parent topic: Ordering Dx and Treating Dx

Add a Modifier

You can add a modifier to any code in the Codes grid. Select the code that you want to add a modifier to. Select the field below the Mod column of the Modifiers grid, and then enter the modifier to be applied. Select the option for Cpt Code. Select the field below the Cpt Code Ind column. A picklist of codes opens. If a modifier is needed, do the following: Due to billing two therapies together, choose the code for the additional therapy – the one NOT selected from the CPT Code grid, and then select OK. On the code you selected from the CPT Code grid (for example, for a chiropractor who needs to apply an AT modifier to CMT codes and the payer is Medicare), select that same code from the list, and then select OK. Select the field below Financial Category column. A picklist of financial categories opens. Select a financial category from the picklist, and then select OK. Add a Treating Diagnosis

As you review the ordering diagnoses for a therapy patient, the diagnosis you want to apply may not be listed. If the ordering diagnoses do not meet therapy treatment needs, you can add a different treating diagnosis. Note: You can also add treating diagnoses by using Problem Intelligent Medical Objects (IMO). If your practice is licensed for IMO, select the Problem IT button under the grid to open IMO to add assessments. Follow the documentation provided by IMO documentation to use IMO to add assessments. Under Treating Dx, select Add. The Inf Common Assessment window opens. Select the Common Assessments field to open a picklist, select the diagnosis codes page you want to use, and then select OK. The Inf Common Assessment window opens. Select the Common Assessments field to select the diagnosis codes page you want to use, and then select OK. The selected page opens. Patient's Problem List

You can launch the diagnosis search window from a Problem List Pop-Up trigger to display only the diagnoses and ICD codes that have been assigned to the patient. You can then select an ICD code from the list. For example, you may want to associate an existing ICD code with a procedure for billing purposes. Note: In this mode, you cannot search All Diagnoses, Favorites, or Categories; use ICD Legacy Mapping; or view, add, or select ICD synonyms. Enter your search criterion in the text box at the top of the diagnosis search window. You can enter all or part of the clinical description or ICD code or description. Select Search. The search results grid displays the patient problems that match your search criterion. To close the diagnosis search window without making a selection, select Cancel. To select an ICD code, double-click the result row you want, or select the row and select Select. The ICD code is automatically entered in the template field assigned that value.
