The Subjective grid displays patient-reported information that was documented on any of the templates for: Function, Work Function, Lymphedema, Pediatrics, Pelvic Health, and TMD. You can open these templates from the links in the left-hand list. Edit an Encounter Assessment
On the Subjective template, General panel, Encounter Diagnosis section, highlight an assessment within the grid. Click Edit to open the Assessments template. Make any necessary changes and then click Add/Update. Click Save & Close. Parent topic: Encounter Diagnosis

Document and Review Patient Pain for a Physical or Occupational Therapy Visit
You can document the patient's pain levels for the current day, when they are at rest, and when they are active. You can also review the pain history within the case to assess the patient's pain levels over time. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Single Visit template, and then expand the Subjective panel. Enter the pain levels in Today, At Rest, and During Activity. Optional: To review the patient's pain levels over time within the case, select the Pain link. The Pain History template opens with the pain details of all the encounters related to the case. To document the pain experienced during the patient's daily functions, select Function in Subjective Grid to open the Function template. Optional: To view historical pain levels from the Function template, select the Previous Pain Grid link. The Previous Pain Grid template opens with pain data from previous versions. You cannot modify these pain values. New pain l
