View Subjective Comments from a Patient’s Initial Evaluation Visit

If you are tracking a patient's progress, you can review the subjective findings from the patient's initial physical or occupational therapy evaluation. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Single Visit template. In the Subjective panel, select the Initial Eval Subjective link. The Initial Eval Subjective window opens with the date and subjective comments from the patient’s initial evaluation. Parent topic: Single Visit Subjective Panel

Subjective Comments

Use the Comments (Subjective) field to record things such as observations, additional information, instructions, and plans that otherwise do not have discrete data fields. You can type text into the field or add saved phrases from a “My Phrases” set. For more information, see Add Comments. The Review Comments link above the field opens a window where you can review comments entered in all sections of the patient’s chart. You can also review and copy comments that were captured on previous encounters with that patient. For more information, see Review, Modify, and Copy Comments. View Injury History within a Case

If any of the patient's past chief complaints and injury details in the case were documented in NextGen® Therapy Suite versions 3.0.11.0 and earlier, you can view them, but you cannot add, edit, or delete them. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Single Visit template. In the Subjective panel, select the Historical Injury Grid link. Document and Review Patient Pain for a Physical or Occupational Therapy Visit

You can document the patient's pain levels for the current day, when they are at rest, and when they are active. You can also review the pain history within the case to assess the patient's pain levels over time. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Single Visit template, and then expand the Subjective panel. Enter the pain levels in Today, At Rest, and During Activity. Optional: To review the patient's pain levels over time within the case, select the Pain link. The Pain History template opens with the pain details of all the encounters related to the case. To document the pain experienced during the patient's daily functions, select Function in Subjective Grid to open the Function template. Optional: To view historical pain levels from the Function template, select the Previous Pain Grid link. The Previous Pain Grid template opens with pain data from previous versions. Single Visit Subjective Panel

Use the Subjective panel on the NTS Single Visit template to document the patient’s history. The Subjective panel also gives you access to additional templates to document the patient’s limitations, work demands, and specialty subjective information. In the Job Title, Work Status, and Last Worked fields, you can document the patient’s workplace and work status information. Note: These fields share the same values from the Job Title, Work Status, and Last Worked fields on the NTS Homepage template. The Ordering Diagnosis grid pulls data entered on the NTS Homepage template. To update Ordering Diagnosis information, open the NTS Homepage template. The Surgery and Past Medical History grids display any history that was captured in a NextGen Healthcare application for the patient. You can edit past medical history and add family and social history. Document Subjective Information for a PT/OT Screening

You can document subjective information for screening such as the patient's chief complaints, the date and mechanism of injury, alleviating and aggravating factors, symptoms, and previous medical intervention. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template, and then in Visit Type, select Screening. Select the Screening tab to open the NTS Screening template. Expand the Subjective panel. Enter the onset date or date of injury and, in Subjective Comments, document the patient's chief complaints.
