Add Diagnosis Codes and Spacers to an Assessment Page

You can add spacers and diagnosis codes with their descriptions to an assessment page.

  1. On the Specialty Common Assessment Config template, expand the Manage Common Assessments panel.
  2. Select the page to be updated from the *Common assessment page name field.
    The name of the selected page appears in the *Common assessment page name field, and the items available in the selected page appear in the assessment item section.
  3. Select the item number, where you want to add the item.
    The selected item number is filled in the Item # field.
  4. Select Description and code or Spacer from the Item to add field.
    The results are as follows:
    • If you select Description and code, the Description field with the Diagnosis Code Lookup link appears, and the Code and Full description fields appear.
    • If you select Spacer, no additional fields appear.
  5. To add a description and code, do the following:
    1. Select the Diagnosis Code Lookup link.
      The diagnosis search window opens.
    2. Enter the keyword in the search field, and select Search.
      The diagnoses that contain the keyword appear.
    3. Select the desired diagnosis from the list, and select Select.
      The diagnosis search window closes, and the details of the selected diagnosis are filled in the Description, Code, and Full description fields.
    4. Modify the display name of the code in the Description field as needed.
  6. Select Add/Update.
    The item is added and appears for the selected item number.
