Add a Surgical Procedure

To open the IORTS Px Xpo template, do one of the following: Under the following grids, click Add: Orthopedic Surgical History grid on the Intake template Surgical History grid on the Intake template Procedures grid on the Surgery Coding template On templates where you are adding procedures to diagnoses/assessments (for example, the Single or Multi Fx template), click the Procedure Codes button. If the Procedure Page field is blank or you want to select a different value, click the Procedure Page field, and select a value from the list and click OK. IORTS Procedure Page window Select a procedure code. If picklists open, make any selections necessary to refine the procedure description: If the IORTS Side picklist opens, select the side or bilateral, or select the blank at the top of the picklist to continue without selecting the side. Surgical History of Patients

You can document a patient's surgical history and orthopedic surgical history. You can use the following sections on the Intake template to document and view the surgical procedures performed for the patient. Orthopedic Surgical History. You can document and view the orthopedic-specific procedures performed for the patient. Surgical History. You can document and view all other surgical procedures performed for the patient. These sections are shared with several templates of the NextGen® Orthopedic Suite, and the grids in these sections display the date of surgery, global date, side, and description. The grids are sorted by DOS column, and the date of surgery can be entered in the MM/DD/YYYY, MM/YYYY, or YYYY formats. To enter a global date, the date of surgery must be in the MM/DD/YYYY format. Add Medical History

You can add medical history such as diseases, disorders, and surgical procedures of a patient. On the appropriate template, in the Medical History section, click Add. The Past Medical History template opens. Past Medical History template Note: By default, the Specialty field displays Orthopedics. In the Medical and Surgical panels, do the following steps for the options without a link. Select the desired check box. The details Details icon icon and a blank date field appear. Click the date field. The Date Selection window opens. Click the calendar icon, and select the date when the disease or disorder first appeared or when the surgery is performed for the patient. Note: You can also enter the date in the field. To add the documented details to the grid in the Past Medical History panel, click Add To Grid. Add Surgery Information to the NTS Homepage Template

You can document the patient's past and upcoming surgeries and indicate whether they pertain to the injury you are treating. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Homepage template, and then select a visit type. Below the Surgery grid, select Add to open the NTS Surgery template. Select Surgery Description, and then do one of the following in the Surgeries list, do one of the following: Select a surgery name, and then select OK. Select the first blank line, select OK, and then enter a custom description. Enter the surgery date and CPT4 code. Select Add. In the confirmation message, do one of the following: If the surgery is related to today’s treatment, select Yes. The surgery information appears in both the Surgical History and Surgery Related to Therapy Episode grids. If the surgery date occurs after the patient completes treatment, the numbers in the Days and Weeks columns appear as negative numbers. If the surgery

Diagnosis/Procedure Information Panel

Procedures and diagnoses pull to the grids in the Diagnosis/Procedure Information panel from the Surgery Sched template. You can add additional procedures and diagnoses to the grids. Procedures and diagnoses entered from the Surgery Sched template cannot be removed, but do not have to be billed for, if not performed. Additional features in this panel include the following: Do not send to Histories– enables you to submit charges for the procedure(s) without sending the procedure(s) to the Orthopedic Surgical History grid. This would prevent providers from tying this procedure to a post-op visit. Submit to History- if you are awaiting sign off, but you want to enter the procedure in the Orthopedic Surgical History grid, click the Submit to History button. This sends the procedure to the Orthopedic Surgical History grid, so providers can tie it to a post-op visit prior to charges being submitted. When ready to submit charges, you can return to this template and click Submit to Superbill.

Add Surgery Information to an Audiology Visit

You can document the patient's past and upcoming surgeries and indicate whether they pertain to the injury you are treating. Open a patient record, and then in Encounter History, expand a case and select an encounter. Open the NTS Audiology Single Visit template, and then select a visit type. In the Subjective panel, select Surgery Hx to open the NTS Surgery template. The Surgical History grid displays any documented past surgeries for the patient. The Surgery Related to Therapy Episode grid displays surgeries related to the current episode of care. Select Surgery Description, and then do one of the following in the Surgeries list: Select a surgery name, and then select OK. Select the first blank line, select OK, and then enter a custom description. Enter the surgery date and CPT4 code. Select Add. In the confirmation message, do one of the following: If the surgery is related to today’s treatment, select Yes. The surgery information appears in both the Surgical History and Surgery Rel
