Manage Surgical History of Patients

On the appropriate template, in the Surgical History or Orthopedic Surgical History section, select the procedure to be edited or removed from the grid. Surgical History grids To edit the selected procedure, do the following: Click Edit below the corresponding grid. The IORTS Px Edit Detail window opens. IORTS PX Edit Detail window Make the desired changes. Note: The CPT4 field is read-only. The Global Date field is automatically filled based on the surgery date for the procedures documented in the patient history sections on the Surgery Coding, Single Fx, and Multi Fx templates. Click Update. The updated details appear in the grid. To remove the selected procedure, click Remove below the corresponding grid, and then select Added in Error from the list. The procedure is removed from the grid. Note: You must be on the encounter where the procedure is added, to remove the selected procedure. Add an Unlisted Procedure Code

You can add an unlisted procedure code from the IORTS Px Xpo template. This will be a generic code by body part or scope for a procedure for which there is no specific procedure code. CAUTION: To avoid mis-coding, before you use this functionality, run a description search for the body part, with the Search Anywhere button selected, and scroll through the results, to be certain there is no existing code for the procedure. Unlisted Procedure section on the IORTS Px Xpo template In the Unlisted Procedure section, click the Code field. A picklist opens for you to choose the generic code by body part, or scope. Select the code from the list. Select unlisted procedures This enters the code in the Code field and closes the picklist. Type a description in the Description field. Click Add. An alert appears asking you to confirm that no code exists for this procedure. Click Yes, if you are sure that there is no existing code for this procedure. Search for a Procedure Code

You can search for a procedure code to add it. On the IORTS Px Xpo template, click the Search button. The IORTS Px Search Pu template opens. Do one of the following: To search by code, type the code into the Code field. IORTS Procedure Search window To search by description, enter the description in the Description field. Note: When searching by description, if you check the Search anywhere check box, the list returns all procedure descriptions that have the typed character string anywhere in the name. Keyword search on the IORTS Procedure Search window The grid list is filtered as you type. For example, when you type “25” in the Code field, the list displays all codes that begin with “25”. Surgical History of Patients

You can document a patient's surgical history and orthopedic surgical history. You can use the following sections on the Intake template to document and view the surgical procedures performed for the patient. Orthopedic Surgical History. You can document and view the orthopedic-specific procedures performed for the patient. Surgical History. You can document and view all other surgical procedures performed for the patient. These sections are shared with several templates of the NextGen® Orthopedic Suite, and the grids in these sections display the date of surgery, global date, side, and description. The grids are sorted by DOS column, and the date of surgery can be entered in the MM/DD/YYYY, MM/YYYY, or YYYY formats. To enter a global date, the date of surgery must be in the MM/DD/YYYY format. Add a Procedure on the Surgery Coding Template

To add a procedure, do one of the following: If using IMO, click the Procedure IT button to launch the IMO template. Click the Procedure Codes button below the Procedures grid to launch the IORTS Px Xpo template. Click on procedures to be billed in the grid. When you click on a procedure, it moves to the coding grid and a check mark appears next to it in the Procedures grid. Only the procedures with a check mark in the Procedures grid and that are in the coding grid can be submitted to Superbill or the patient’s histories. Add Surgical Procedure Codes

You can add up to eight procedures using the Procedures Code button and the IORTS Px XPO template, or five procedures using the Procedure IT button and IMO functionality. If you enter more than five procedure codes, only five appear in the fields on this page. However, all procedures appear on patient documentation. Note: Selecting the P check box to the left of a procedure code fields indicate that the procedure is possibly going to be performed, but may not be, based on the surgeon’s discretion or insurance authorization. To add procedures, do one of the following: To add procedures from the IORTS Px XPO template, click the Procedure Codes button. The IORTS Px XPO template opens on a default procedure page based on practice or provider. If you are licensed for IMO functionality, click the Procedure IT button to add procedure codes.
