You can enter general information about the problems in the Reason for Appointment section.
From the Home template, click the Pre Reg link.
In the Appointment panel, click What is the problem and select from the multiple lists.
Select a side, or the top blank side, if side is not applicable.
Select a location (body part or region).
Select the complaint.
Select Yes or No for Was this an accident or injury? to specify if this complaint is the result of an auto accident, is a liability, or is a workman’s compensation visit.
Select the Do you plan on using you VA benefits, if the patient plans to use these benefits
Enter the Provider Information:
Location
Provider Requested
Have you seen another Dr. for this problem?
Select what appointment type is needed.
Enter any additional information that may be required for that appointment type.
