Track Forms
Double-click in the Forms Tracking grid to open the Patient Request for Form Completion template. Patient Request for Form Completion template Enter in information regarding the type of form, number of pages, whether, how, and when it was received, and to whom it was sent. Use the check boxes to track attempts made to contact the patient. Fill in the fields for the names and addresses to where the form was returned. Click Add to Worklog to add the form to the forms worklog for staff members to work off and keep track of the process. Enter notes about a form, as needed, in the Notes field. Select Template Complete when the form has been completed. Click Save, and click Close. Parent topic: Forms Tracking

Forms Tracking
The Forms Tracking grid displays forms that are being tracked (for example, disability, FMLA, Work Comp). The Patient Request for Form completion template enables form tracking. You can filter the grid to show Completed Forms or Declined Forms. You can task medical records and generate the Medical Record Release document. Med Rec Release Template
Use the Med Rec Release template to enter basic information on for a medical record release. Anything entered on this template appears on the Medical Record Release to Practice document. Med Rec Release template The patient information at the top of the template pulls in from NextGen® Enterprise PM. Click the Forms link, and double-click Med Rec Release from the list. Enter the Doctor or Facility from which the information is being released. Enter the name of the person or facility to whom the records are being released in the Release to field. Check all applicable check boxes for Information to be released. Enter Service Dates, and any restrictions by clicking in the appropriate fields. Select the Restrictions option. Once all information is entered, click the MR release to Practice document icon to generate the document. Parent topic: Forms

Medical Records Request and Disclosure Log
The Medical Records Request and Disclosure Log grid displays records that have been requested and to whom they have been disclosed. Double click the Medical Records Request and Disclosure Log grid to open the IORTSreq_disclosure template. IORTS Request Disclosure window Enter in the date the request was received in the Date Received field. Click the Med Records Release Form field and select whether the patient signed or refused to sign. Click the Records Requested field and select all records requested from the multi-select picklist. Select the format of the records disclosed (Paper, Electronic, etc.). Enter who disclosed the records. Enter the Date of Disclosure. Enter the contact information for the person or entity to whom the records were disclosed. Select Completed. The user name enters the field, with a date and timestamp. Click Save.
