You can update a previously documented problem that appears in the
Problem List
.
In the
Problem List
panel, select a problem listed in the grid.
The fields below the grid display details for the problem.
Update the required changes in the fields below the grid.
Click
Add/Update
to save the changes.

Add Problem to the Problem List
To open the Problems window, do one of the following: In the Problem List panel, click Add/Update Click Problems in the patient information bar. The Problems window opens. Enter a problem in the diagnosis search window and click Search. Highlight the problem from the list and click Select. The system fills the associated values in the Fully Specified SNOMED® Description, Problem Description, ICD Description and ICD Code fields in the Problem List panel. For all new problems, Mark as Chronic is selected by default. Enter the Clinical Status. Enter the Onset Date. Enter the Resolved Date. If the problem is not chronic, do one of the following: Select Secondary Condition to mark the problem as a secondary condition. Select Marked as Tracked Problem to add the selected problem to the tracked problem list. To add the problem to the assessment list, select Add to Today’s Assessments. In Problem Notes, enter notes specific to the problem. Click Add/Update to add the problem to the problem lis

Update a Problem
You can modify a documented problem. Open NextGen® Enterprise EHR. Open the Histories template. In the Problem List panel, select a problem. Patient's Problem List
You can manage a patient's problems. You can perform the following actions in the Problem List panel. Record, modify, and retrieve a patient's clinical problem list electronically to meet Quality Reporting requirements Document the patient's chronic conditions and ongoing clinical problems View problems from past encounters Add problem details Problem List You can also indicate if a patient's documented problem is completed and reviewed, and if there is any active problems. You can select the Include Resolved check box to include the resolved problems. Open Problem List You can access the problem list to document and view a patient's problem history. Patient's Problem list
You can do the following in the Problem List panel: Electronically record, modify, and retrieve a patient's clinical problem list to meet Quality Reporting requirements Document a patient's chronic conditions and ongoing clinical problems. View problems from past encounters Update problem details To indicate that the review of the patient's documented problem is complete, the provider must select the Reviewed check box. Update a Problem Note
Select the patient's encounter that contains the problem. Select a problem in the Problem List. Do one of the following: Right-click the problem, and then select View/Add Notes. On the toolbar of the Problem data entry area, select View/Add Notes. The Problem Notes window displays the existing notes. Problem Notes Select the note in the Problem Notes window, and then select Update Note. The Edit Problem Note window displays the current note. Update the note as required. Documenting Problem Notes
You can document a problem note for a patient problem. If you add a note to a problem, the note appears when the corresponding problem is selected in the grid. If notes are not added, you can add a note to the problem, and the note appears in Problem Notes. You can use Most Recent Note by Current User to view authored notes, and Most Recent Note to view notes entered by other users. The notes entered by other users are read-only. The View all notes link enables you to view all the problem notes associated with the selected problem. Based on the configuration done in Problem List Note on the Practice Configuration template, you can set a default view option to filter notes.
