Filter the Patient's Diagnosis History

The Dx History section of the Assessment/Plan - OPH template lists a recent history of diagnoses for the patient. dx history window In the Dx History section, select one of the following options to filter the patient's diagnosis. Filter the Patient's Clinical Problems

The Clinical Problems section of the Assessment/Plan - OPH template lists tracked problems and chronic conditions for the patient. clinical problems section On the Assessment/Plan - OPH template, expand the A/I/P Details panel and ensure that the DX HX/My Favourites option is selected. Note: The DX HX/My Favourites option is selected by default. For more information on changing the default option, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide for NextGen® Adaptive Content Engine. Select the Chronic check box to view the patient's chronic conditions. Select the Tracked check box to view the patient's tracked problems. To learn how to mark a problem as chronic or tracked, go to Add Problem to the Problem List. Select the No Active check box to view the patient's problems that are no longer active. Add a New Assessment on the Common Assessments Template

On the Assessment/Plan - OPH template, click the Common Assessment link. The Common assessment template opens displaying common assessments for Ophthalmology. The template displays common assessments for Retina for a provider configured as a Retina specialist on the Eye Configuration practice template. Filter the My Favorites List

Ophthalmic providers can organize their lists of favorite assessments in different lists for ease of use using Favorites Category. Click the drop-down list for Favorites Category and select the appropriate category of saved favorite diagnoses to display. Select All to display all. The Filter field allows you to narrow your favorites list to what you are specifically looking for. Type in a few letters and the list refreshes to display those saved favorite assessments with descriptions that start with those letters. Parent topic: My Favorites Assessments

Enter Assessments for Today's Visit

Assessments can be added to Today's Assessments in several ways, including by selecting from the patient's clinical problems or diagnosis history, the provider's list of favorite assessments, the Common Assessments template, or the diagnosis search window. New assessments can be added to various lists for the patient or the provider's My Favorites list, and they can be sorted to display in a preferred order. If a technician is acting as a scribe for the provider, select a name from the Scribe field. Review Today's Concern/Reason For Visit. Review Today's Assessments

Ophthalmic providers can review previously documented diagnoses/assessments that were selected for the patient for this encounter listed in the Today's Assessments grid in the Diagnosis panel on the Finalize - OPH template. CAUTION: A diagnosis added or modified on the Finalize - OPH template does not appear in the Impression/Plan Summary grid. However, it appears in the Diagnosis History and in the Chart Note.
