Document an Ocular Procedure

You can document Ophthalmology procedures in detail on the Record Patient Procedure - OPH template.

  1. Access the Record Patient Procedure – OPH template .
  2. At the top of the template, select the Eye involved in the procedure: OD, OS, or OU.
  3. Click the Dx description field to enter a diagnosis description.
    The diagnosis search window opens.
  4. Search for and select the diagnosis, and then click Select.
    The system opens the Procedure window. The diagnosis details fill the Dx description and Dx code fields.
  5. Search for and select the procedure.
    The selected procedure description and its associated code appear in the Px description and Px code fields.
  6. Enter a diagnosis Status.
  7. Select up to four billing modifiers using the Modifier fields 1 through 4.

    If you are not going to bill for the procedure, this step is optional.

  8. Select a Category option for the ocular condition.
  9. To exclude the procedure from billing, select the Suppress billing check box.
    This sets a flag to NextGen® Enterprise PM that the procedure is not to be billed.
  10. To add the procedure to the Histories Master, select the Add to histories master check box.
  11. Click the Add button at the lower right of the Procedures Details section of the template to add the procedure to the Procedures grid and to the Histories Master grid.
  12. Click Return.
