Document an Ocular Procedure
You can document Ophthalmology procedures in detail on the Record Patient Procedure - OPH template.
- Access the Record Patient Procedure – OPH template .
 - At the top of the template, select the Eye involved in the procedure: OD, OS, or OU.
 -  Click the Dx description field to enter a diagnosis description. 
   The diagnosis search window opens.
 - Search for and select the diagnosis, and then click Select.
   The system opens the Procedure window. The diagnosis details fill the Dx description and Dx code fields.
 - Search for and select the procedure. 
   The selected procedure description and its associated code appear in the Px description and Px code fields.
 - Enter a diagnosis Status.
 - Select up to four billing modifiers using the Modifier fields 1 through 4. 
   
If you are not going to bill for the procedure, this step is optional.
 - Select a Category option for the ocular condition.
 - To exclude the procedure from billing, select the Suppress billing check box.
   This sets a flag to NextGen® Enterprise PM that the procedure is not to be billed.
 - To add the procedure to the Histories Master, select the Add to histories master check box.
 - Click the Add button at the lower right of the Procedures Details section of the template to add the procedure to the Procedures grid and to the Histories Master grid.
 - Click Return.
