Ocular Procedures
You can use the Record Patient Procedure – OPH template to document procedures recently performed by the current provider. The Record Patient Procedure – OPH template enables you to document detailed procedure and billing information and uploads the procedure information to both the Procedures module and the Histories table. Document Ocular History
You can document new ocular history on the Ocular History - OPH template. On the Histories tab, click the Add in the Past Ocular panel to open the Ocular History – OPH template. ocular history - oph Select an ocular category. The category determines the specific lists of choices for disease and procedure or management that open. Enter the following details, as needed: Disease - the associated SNOMED code automatically fills the corresponding field Eye Date Dx Procedure - the associated SNOMED code automatically fills the corresponding field Eye - eye involved in the procedure Global end Date – refers to the global time period for post-operative days included as part of the billing for the procedure; this is the last date of the global time period and based on the selected procedure date and the global time period defined by the practice Proc Date - procedure date Surgeon Location Outcome Click Save. Update Existing Ocular History
You can update previously documented ocular history. On the Histories tab, select a row in the Past Ocular History grid and click Edit. The Ocular History – OPH template opens and displays details for the highlighted row. Make any applicable changes to the information displayed in the fields. Click Save and then click Close. Document an Ocular Procedure
You can document Ophthalmology procedures in detail on the Record Patient Procedure - OPH template. Access the Record Patient Procedure – OPH template . At the top of the template, select the Eye involved in the procedure: OD, OS, or OU. Click the Dx description field to enter a diagnosis description. The diagnosis search window opens. Search for and select the diagnosis, and then click Select. The system opens the Procedure window. The diagnosis details fill the Dx description and Dx code fields. Search for and select the procedure. The selected procedure description and its associated code appear in the Px description and Px code fields. Enter a diagnosis Status. Select up to four billing modifiers using the Modifier fields 1 through 4. If you are not going to bill for the procedure, this step is optional. Select a Category option for the ocular condition. To exclude the procedure from billing, select the Suppress billing check box. This sets a flag to NextGen® Enterprise PM that the

Past Ocular History
You can add, update, and view a history of ocular conditions, procedures, and treatments for the patient on the Past Ocular History panel. You can use the fields above the grid to document that the patient has a prosthetic eye and to indicate the type of review performed. Access the Procedures - OPH template. Do one of the following to access the Procedures - OPH template: From the Intake - OPH, Prelim Exam - OPH, and Provider - OPH templates, click the Procedure link. From the Additional Navigation section at the bottom of the Prelim Exam - OPH and Provider - OPH templates, click the Procedures link. From the left navigation, click the Procedures link.
