Click the Consent Forms link.
The Consent Forms - OPH template opens.
The patient’s name appears at the top of the template. The Consent date and Time fields display the current system date and time when the template opens. The Condition(s) have been explained by Dr.: field displays the currently selected provider’s name. The Procedure date and Time fields display the current system date and time by default.
