You can use the Past Vaccine Entry Form to record vaccinations received outside the practice.
 
 
  - Open NextGen® Enterprise EHR, and select a patient from the Patient Lookupwindow.
 
  - Select or create an encounter from Encounter History.
 
  - Open the Immunizations module.
 
  - Select Historical.
   
    Note: If the selected encounter is not created for that day, a message appears prompting you to do one of the following:
    
 
    
   
    - To select a different encounter, select Cancel, and then select an encounter from the Encounter History tab.
 
    - To continue with the currently selected encounter, select No.
 
    - To create an encounter, select Yes.
 
   
   
    The Past Vaccine Entry Form window opens.
   
 
  - To select a vaccine from the past vaccines list, select a date range from the By Category list, and then select a displayed vaccine.
 
  - To search for a vaccine, do the following:
   
    - Select Search.
 
    - Enter the vaccine name, CVX code, CPT4® code, or the CPT4 description, and then select Find.
 
    - Select Search all vaccines to expand the search for vaccines not included in the inventory.
 
    - From the displayed list, select a vaccine and then select OK.
 
   
    
  - Under Selected Vaccine Details, enter the date of administration of the vaccine, and select a Source from the list.
 
  - To edit the record of a previous vaccine, on the Past Vaccine Entry Form window, select the Vaccine Edit tab and enter the details.
 
  - Select Save and Close.
   
    The recorded vaccine is updated in the Immunizations tab.
